目的:应用三维超声探索乳腺结节良恶性的诊断及鉴别良恶性的价值。方法50例乳腺结节患者进行三维超声诊断,对检查结果进行冠状面结节的边缘特点、周围组织情况及重建彩色血流图分析。结果50例乳腺结节病例分为恶性组与良性组,两组病例在结节边缘、汇聚征出现、血流分布方面差异有统计学意义(P<0.05)。分别以边缘不清、汇聚征、血流丰富程度作为诊断恶性的标准,各自的敏感性为83.2%、82.5%、81.5%;各自的特异性为72.4%、85.4%、72.3%;各自的准确性为78.0%、86.0%、68.5%。综合三者,敏感性为82.4%,特异性为76.7%,准确性为77.5%。结论三维超声对乳腺结节良恶性的鉴别诊断具有一定的临床价值,可能是检测乳腺结节良恶性的有效手段。

목적:응용삼유초성탐색유선결절량악성적진단급감별량악성적개치。방법50례유선결절환자진행삼유초성진단,대검사결과진행관상면결절적변연특점、주위조직정황급중건채색혈류도분석。결과50례유선결절병례분위악성조여량성조,량조병례재결절변연、회취정출현、혈류분포방면차이유통계학의의(P<0.05)。분별이변연불청、회취정、혈류봉부정도작위진단악성적표준,각자적민감성위83.2%、82.5%、81.5%;각자적특이성위72.4%、85.4%、72.3%;각자적준학성위78.0%、86.0%、68.5%。종합삼자,민감성위82.4%,특이성위76.7%,준학성위77.5%。결론삼유초성대유선결절량악성적감별진단구유일정적림상개치,가능시검측유선결절량악성적유효수단。
Objective To explore the application value of three‐dimensional ultrasound in the differential diag‐nosis of benign and malignant breast nodules .Methods 50 cases of breast nodules underwent the three‐dimensional ultrasound examination .The examinatin results were performed the color flow mapping analysis on the coronal plane nodules edge features ,surrounding tissue situation and reconstruction .Results 50 cases of breast nodules were divid‐ed into the malignant group and the benign group .The differences in the aspects of the nodular edge ,appearance of convergence sign and blood flow distribution had statistically significant differences between the two groups(P<0 .05) .With the unclear edge ,convergence sign and plenty of blood flow as the standard for diagnosing malignant nodules ,their corresponding sensitivities were 83 .2% ,82 .5% and 81 .5% ;specificities were 72 .4% ,85 .4% and 72 .3% and accuracies were 78 .0% ,86 .0% and 68 .5% respectively .Combining these 3 indicators ,the sensitivity was 82 .4% ,the specificity was 76 .7% and the accuracy was 77 .5% .Conclusion The three‐dimensional ultrasound has certain clinical value in the differential diagnosis of benign and malignant breast nodules ,and may be an effective means for detecting benign and malignant breast nodules .
