目的 评价呼出气一氧化氮(fractional exhaled nitric oxide,FeNO)在检测诊断咳嗽变异性哮喘(cough variant asthma,CVA)方面的准确性及最佳界值.方法 收集2012年10月至2013年10月在苏州大学附属第一医院门诊因慢性咳嗽就诊的172例患者,其中80例最终被诊断为CVA患者,92例为非CVA患者,分为CVA组和非CVA组.所有患者均使用Astrograph气道反应测定仪和FeNO测试仪分别进行激发试验及FeNO浓度检测.根据不同的FeNO值对应的敏感度及特异度绘制受试者工作特征曲线(ROC曲线),评估FeNO诊断CVA的准确性并得出最佳界值.结果 CVA组FeNO值(41.04±18.41)明显高于非CVA组(23.60±16.58)(P<0.01).ROC曲线下面积是0.830(95%CI:0.767~0.893),FeNO诊断CVA的最佳界值为33 ppb,以33 ppb为诊断CVA的敏感度为73.4%,特异度为81.0%,阳性预测值为77.9%,阴性预测值为78.9%,准确度为78.5%.结论 FeNO在诊断CVA方面具有较高的敏感度和特异度,可作为CVA的辅助诊断手段.

Objective Evaluate the accuracy of fractional exhaled nitric oxide (FeNO) determination for the diagnosis of cough variant asthma (CVA) and determine the optimal cut point.Methods Collected 172 cases of chronic cough patients from October 2012 to October 2013 in the First Affiliated Hespital of Soochow University Hospital.Eighty of them were diagnosed with CVA patients and the other 92 were non-CVA,divided into the CVA group and the non-CVA.Astrograph Airway Responsiveness Analyzer and FeNO Tester were used to conduct the excitation experiment and FeNO concentration detection.Based on different FeNO cut point corresponding to specified degree of sensitivity and specificity,the ROC curve was plotted to assess the accuracy of FeNO CVA diagnosis and its optimum cut point.Results FeNO value of CVA group [(41.04± 18.41) ppb] was significantly higher than the non-CVA group [(23.60± 16.58) ppb],and P <0.01.Area under the ROC curve was 0.830 (CI:0.767~0.893),the optimal cut point of FeNO used to diagnose CVA is at 33 ppb,by which the value of sensitivity was 73.4 % and specificity 81.0 %,positive predictive value 77.9 %,negative predictive value 78.9%,accuracy 78.5%.Conclusions FeNO used in the CVA diagnosis shows higher sensitivity and specificity,and it can be used as auxiliary diagnosis of CVA.
