目的:探讨皮下注射草甘膦致中毒病例的临床特点及治疗对策。方法对1例右侧臀部皮下注射草甘膦致中毒病例的临床资料进行综合分析。结果皮下注射草甘膦后患者出现头晕、乏力、发热(体温39℃)和血压下降(由105/60 mmHg降至80/50 mmHg)等全身中毒反应,同时草甘膦注射部位局部软组织出现红肿疼痛,并出现小水泡。患者经大量补液、行注射部位皮肤切开引流术、抗感染及对症支持治疗10 d 后痊愈出院,注射部位无出现组织坏死,无重要器官功能损害。结论对于局部注射农药引起中毒的患者,在全身治疗的同时,作局部切开引流以清除组织内残留农药是治疗的关键。

목적:탐토피하주사초감련치중독병례적림상특점급치료대책。방법대1례우측둔부피하주사초감련치중독병례적림상자료진행종합분석。결과피하주사초감련후환자출현두훈、핍력、발열(체온39℃)화혈압하강(유105/60 mmHg강지80/50 mmHg)등전신중독반응,동시초감련주사부위국부연조직출현홍종동통,병출현소수포。환자경대량보액、행주사부위피부절개인류술、항감염급대증지지치료10 d 후전유출원,주사부위무출현조직배사,무중요기관공능손해。결론대우국부주사농약인기중독적환자,재전신치료적동시,작국부절개인류이청제조직내잔류농약시치료적관건。
Objective To analyze the clinical characteristics and treatment measures on a case of glyphosate poisoning caused by hypodermic injection .Methods The clinical data of a patient who suffered from glyphosate poisoning caused by hypodermic injection on the right buttock was comprehensively analyzed . Results After hypodermic injection with glyphosate, the patient was found to have dizziness , fatigue, fever (39 ℃), decreased blood pressure (from 105/60 to 80/50 mmHg) and other systemic toxic reactions , meanwhile the local soft tissue in injection area got swelling and pain and then blisters appeared on the skin of right buttock .The patient got recovery and discharged 10 days after treatment with a series of symptomatic support cure , such as a large amount of fluid infusion , anti infection , incision and drainage to the injection site skin.Tissue necrosis of the injection sites and important organ damage were not found .Conclusion The key point to achieve successful rescue for the poisoned patients with local injection pesticides is the operation of local incision and drainage for removing toxin from the tissue while the patients receive general treatment at the same time .
