目的：观察黄芪多糖配合心理护理对直肠癌术后患者细胞免疫及血清炎性因子表达的影响。方法选取160例直肠癌患者随机分为常规护理组、心理护理组、黄芪多糖组和黄芪多糖配合心理护理组(联合组)。采用流式细胞术对患者外周血T 细胞亚群进行检测。应用实时定量PCR法和ELISA法检测各组患者血清炎性细胞因子 mRNA表达水平和蛋白浓度。结果与常规护理组相比,黄芪多糖组患者手术后3项免疫指标( CD4+、CD4+ /CD8+及NK)升高,血清中TNF-α的蛋白含量和mRNA表达水平明显下降；与常规护理组相比,心理护理组患者治疗后NK升高,TNF-α、IL-6和IL-8的蛋白含量和mRNA表达水平明显下降；与常规护理组相比,联合组患者治疗后细胞免疫4项指标( CD3+、CD4+、CD4+ /CD8+及 NK)升高,CD8+下降,血清中 TNF-α、IL-6、IL-8和IL-12的蛋白含量和mRNA表达水平明显下降；与黄芪多糖组相比,联合组患者术后细胞免疫2项指标( CD3+和NK)升高,TNF-α和IL-12的蛋白含量和mRNA表达水平明显降低。结论黄芪多糖配合心理护理不仅调节细胞免疫功能,还能降低直肠癌患者血清中炎性因子的表达水平,从而增强直肠癌患者免疫力。
목적：관찰황기다당배합심리호리대직장암술후환자세포면역급혈청염성인자표체적영향。방법선취160례직장암환자수궤분위상규호리조、심리호리조、황기다당조화황기다당배합심리호리조(연합조)。채용류식세포술대환자외주혈T 세포아군진행검측。응용실시정량PCR법화ELISA법검측각조환자혈청염성세포인자 mRNA표체수평화단백농도。결과여상규호리조상비,황기다당조환자수술후3항면역지표( CD4+、CD4+ /CD8+급NK)승고,혈청중TNF-α적단백함량화mRNA표체수평명현하강；여상규호리조상비,심리호리조환자치료후NK승고,TNF-α、IL-6화IL-8적단백함량화mRNA표체수평명현하강；여상규호리조상비,연합조환자치료후세포면역4항지표( CD3+、CD4+、CD4+ /CD8+급 NK)승고,CD8+하강,혈청중 TNF-α、IL-6、IL-8화IL-12적단백함량화mRNA표체수평명현하강；여황기다당조상비,연합조환자술후세포면역2항지표( CD3+화NK)승고,TNF-α화IL-12적단백함량화mRNA표체수평명현강저。결론황기다당배합심리호리불부조절세포면역공능,환능강저직장암환자혈청중염성인자적표체수평,종이증강직장암환자면역력。
Objective To investigate the effect of astragalus polysaccharide ( APS) combined with psycholog-ical nursing on cellular immunity and expression of proinflammatory cytokines in patients after rectal cancer opera-tion. Methods 160 rectal cancer patients were randomly divided into routine nursing ( RN) group,APS group,psycho-logical nursing ( PN ) group and combined group ( APS combined with psychological nursing ) , 40 patients in each group. The T cell subsets after operation of 160 patients were detected by flow cytometry. ELISA was used to detect the contents of serum proinflammatory cytokines protein;Real-time PCR was used to detect the expression levels of cyto-kines mRNA. Results Compared with RN group,the CD4 +,CD4 + /CD8 +and NK indexes in APS group increased significantly,the contents and expression levels of TNF-αprotein and mRNA in APS group decreased significantly(P<0. 05);the NK in PN group increased significantly(P<0. 05);the contents and expression levels of TNF-α,IL-6 and IL-8 proteins and mRNA in PN group decreased significantly(P<0. 05),the CD3 +,CD4 +,CD4 + /CD8 +and NK in combined group increased significantly,the CD8 +indexes in combined group decreased significantly,the contents and expression levels of TNF-α,IL-6 and IL-8 proteins and mRNA in combined group decreased significantly,there were significant differences (P<0. 05). Compared with APS group,the CD3 +and NK in combined group increased signifi-cantly,the contents and expression levels of TNF-αand IL-12 proteins and mRNA in combined group decreased signifi-cantly. Conclusion APS combined with psychological nursing can not only regulate the immuniological function,but also reduce the expression levels of inflammatory cytokines in serum of rectal cancer patients,so as to improve the im-munity of patients.
