目的：探讨临床药师如何做好药学监护工作。方法阐述临床药师对1例食管癌术后并发感染患者实施药学监护的全过程。根据患者的个体情况，临床药师与医生协商制定了化疗方案，并就化疗过程中出现的骨髓抑制、感染及皮肤毒性反应给出了相应的用药方案。结果临床药师熟练掌握药物作用机制和不良反应相关知识，与临床医生协作，根据患者疗效及时调整用药，最终顺利完成周期治疗。结论临床药师应熟悉药物作用机制和不良反应相关知识，提供专业的药学服务。
목적：탐토림상약사여하주호약학감호공작。방법천술림상약사대1례식관암술후병발감염환자실시약학감호적전과정。근거환자적개체정황，림상약사여의생협상제정료화료방안，병취화료과정중출현적골수억제、감염급피부독성반응급출료상응적용약방안。결과림상약사숙련장악약물작용궤제화불량반응상관지식，여림상의생협작，근거환자료효급시조정용약，최종순리완성주기치료。결론림상약사응숙실약물작용궤제화불량반응상관지식，제공전업적약학복무。
Objective To discuss how to perform pharmaceutical care of clinical pharmacist. Methods The whole pharmaceutical care process on a esophageal carcinoma patient with postoperative infection was expounded. Based on the patient’ s individual situation,the clinical pharmacist talked with doctors and worked out a chemotherapy regimen. Moreover,the relevant drug regimens were prepared to alleviate the adverse reactions,such as bone marrow suppression,infection and dermal toxicity. Results The medication was timely adjusted based on the therapeutic effi-cacy. The patient successfully completed the cycle of chemotherapy. Conclusion Clinical pharmacist should be famil-iar with the mechanisms of drug action and adverse reactions,and provide better pharmaceutical care through the coordi-nation with doctors.
