Atypical lymphocyte%CMV%EBV
目的:探讨患儿感染CMV、EBV后,异型淋巴细胞数量与CMV-IgM、CMV-IgG、CMV-DNA、EBV-IgM、EBV-IgG水平,以及与CMV-DNA和EBV-DNA之间的相关性,为患儿治疗CMV和EBV提供实验室依据。方法收集异型淋巴细胞增多(阳性:异型淋巴细胞>3%)儿科临床病例95例,采用ELISA法检测患者血清中CMV-IgM、CMV-IgG、EBV-IgM、EBV-IgG水平,Real-time PCR法检测患者CMV-DNA 和EBV-DNA含量,瑞吉染色计数异型淋巴细胞。结果95例异型淋巴细胞阳性患者体内CMV-IgM和EBV-IgM阳性率达1.1%和5.3%;CMV-IgG和EBV-IgG阳性率为52.6%和31.6%;PCR结果显示CMV-DNA和EB-DNA阳性率分别为17.9%和22.1%。其中50例CMV-IgG检测阳性的患者CMV-DNA阳性率高达30%;30例EBV-IgG检测阳性的患者EBV-DNA阳性率高达36.7%;5例EBV-IgM检测阳性的患者仅1例EB-DNA检测为阴性。结论异型淋巴细胞增多与CMV、EBV感染具有一定的相关性,且CMV-IgG和EBV-IgG在诊疗过程中具有一定临床意义。怀疑患儿感染CMV和EBV时,合并异型淋巴细胞、CMV-IgM、CMV-IgG、CMV-DNA、EBV-IgM、EBV-IgG以及EBV-DNA水平联合诊断对临床具有重要意义。

목적:탐토환인감염CMV、EBV후,이형림파세포수량여CMV-IgM、CMV-IgG、CMV-DNA、EBV-IgM、EBV-IgG수평,이급여CMV-DNA화EBV-DNA지간적상관성,위환인치료CMV화EBV제공실험실의거。방법수집이형림파세포증다(양성:이형림파세포>3%)인과림상병례95례,채용ELISA법검측환자혈청중CMV-IgM、CMV-IgG、EBV-IgM、EBV-IgG수평,Real-time PCR법검측환자CMV-DNA 화EBV-DNA함량,서길염색계수이형림파세포。결과95례이형림파세포양성환자체내CMV-IgM화EBV-IgM양성솔체1.1%화5.3%;CMV-IgG화EBV-IgG양성솔위52.6%화31.6%;PCR결과현시CMV-DNA화EB-DNA양성솔분별위17.9%화22.1%。기중50례CMV-IgG검측양성적환자CMV-DNA양성솔고체30%;30례EBV-IgG검측양성적환자EBV-DNA양성솔고체36.7%;5례EBV-IgM검측양성적환자부1례EB-DNA검측위음성。결론이형림파세포증다여CMV、EBV감염구유일정적상관성,차CMV-IgG화EBV-IgG재진료과정중구유일정림상의의。부의환인감염CMV화EBV시,합병이형림파세포、CMV-IgM、CMV-IgG、CMV-DNA、EBV-IgM、EBV-IgG이급EBV-DNA수평연합진단대림상구유중요의의。
Objective Probe the correlation of atypical lymphocyte with CMV-IgM, CMV-IgG, CMV-DNA, EBV-IgM, EBV-IgG and EBV-DNA in pediatric patients, provide experimental basis for treatment of CMV and EBV infection in children patients. Methods Collected 95 samples of atypical lymphocyte more than 3% from clinic pediatrics, detected the level of CMV-IgM, CMV-IgG, EBV-IgM and EBV-IgG by ELISA, analyzed the copies of CMV-DNA and EBV-DNA with real time PCR, and the peripheral blood smears were stained by Wright-Giemsa. Results There were 1.1% CMV-IgM, 5.3% EBV-IgM, 52.6% CMV-IgG and 31.6% EBV-IgG positive were discovered in 95 children patients of atypical lymphocyte increased by ELIAS, 17.9%CMV-DNA and 22.1%EBV-DNA were confirmed by PCR, respectively. 30%CMV-DNA positive patients were found at 50 samples with CMV-IgG;36.7%EBV-DNA were found at 30 cases with EBV-IgG. There was only one EBV-DNA negative in 5 casws with EBV-IgM. Conclusion Results showed that atypical lymphocyte increased has correlation with CMV-DNA, EBV-DNA, CMV-IgG and EBV-IgG, it will provide a clinical reference for the diagnosis and treatment of CMV and EB infection.
