XuanJu detoxification gel%pregnancy gingivitis%IL-1β%TNF-α
目的：观察玄菊解毒凝胶治疗妊娠期龈炎临床疗效。方法选取2013年9月至2014年12月在成都中医药大学附属医院口腔科就诊的54例妊娠期龈炎患者的临床资料，根据治疗用药不同分为A、B、C 3组。1％双氧水5 mL／牙＋生理盐水5 mL／牙冲洗治疗的患者纳入A组（18例）；1％双氧水5 mL／牙＋生理盐水5 mL／牙冲洗治疗基础上，联合玄菊解毒凝胶0.5 mL／牙治疗的患者纳入B组（18例）；单纯玄菊解毒凝胶0.5 mL／牙治疗的患者纳入C组（18例）。分别观察：①患者治疗前后龈沟出血指数（ sulcus bleeding index，SBI）、菌斑指数（plaque index，PI）、牙周探诊深度（periodontal pocket depth ，PPD）；②患者治疗前后龈沟液中白细胞介素（ interleukin－1β，IL－1β）、肿瘤坏死因子（ tumour necrosis factor －α，TNF－α）含量；③患者自觉症状、口臭的改善程度。结果①B、C两组治疗前后PI、SBI、IL－1β、TNF－α及自觉症状、口臭指标比较，差异有统计学意义（P＜0.05）。 A组治疗前后仅自觉症状差异有统计学意义（P＜0.05）。②3组在治疗后除PPD以外，其余各指标B组均优于C组和A组（ P＜0.05）。结论玄菊解毒凝胶组方精简，剂型优越，与基础治疗联合应用对于妊娠期龈炎有较好疗效。

목적：관찰현국해독응효치료임신기간염림상료효。방법선취2013년9월지2014년12월재성도중의약대학부속의원구강과취진적54례임신기간염환자적림상자료，근거치료용약불동분위A、B、C 3조。1％쌍양수5 mL／아＋생리염수5 mL／아충세치료적환자납입A조（18례）；1％쌍양수5 mL／아＋생리염수5 mL／아충세치료기출상，연합현국해독응효0.5 mL／아치료적환자납입B조（18례）；단순현국해독응효0.5 mL／아치료적환자납입C조（18례）。분별관찰：①환자치료전후간구출혈지수（ sulcus bleeding index，SBI）、균반지수（plaque index，PI）、아주탐진심도（periodontal pocket depth ，PPD）；②환자치료전후간구액중백세포개소（ interleukin－1β，IL－1β）、종류배사인자（ tumour necrosis factor －α，TNF－α）함량；③환자자각증상、구취적개선정도。결과①B、C량조치료전후PI、SBI、IL－1β、TNF－α급자각증상、구취지표비교，차이유통계학의의（P＜0.05）。 A조치료전후부자각증상차이유통계학의의（P＜0.05）。②3조재치료후제PPD이외，기여각지표B조균우우C조화A조（ P＜0.05）。결론현국해독응효조방정간，제형우월，여기출치료연합응용대우임신기간염유교호료효。
Objective To evaluate the clinical effects of XuanJu detoxification gel in the treatment of pregnancy gingivitis . Methods Clinical data of 54 outpatients with pregnancy gingivitis in Affiliated Hospital of Chengdu University of TCM from Sep.2013 to Dec.2014 were selected.All patients were divided into group A , B and C according to different therapeutic methods with each group 18 cases.Group A:Washing affected teeth with 5 mL 1%H2O2 and 5 mL saline per teeth.group B:Besides the therapy above, applying XuanJu detoxification gel ( 0.5 mL per teeth ) .Group C: Using the gel only .The treatment indicators were observed: ①Sulcus bleeding index (SBI), plaque index (PI), periodontal pocket depth (PPD).②Nterleukin-1β(IL-1β), tumour necrosis factor-α( TNF-α) of gingival crevicular fluid .③ subjective symptoms , the improvement of halitosis .Results ① There were significant differences in PI , SBI, IL-1β, TNF-α, subjective symptoms, and the degree of halitosis before and after treatment of group B and group C (P<0.05).It found that only subjective symptoms had significant difference before and after treatment in group A.②All indicators in Group B were superior to group A and group C after treatment except PPD (P<0.05).Conclusion XuanJu detoxification gel combined with basic treatment has a good effect in the treatment of pregnancy gingivitis which has the advantages of simple prescription and superior dosage form .
