目的:研究颅脑外伤神经内分泌的功能障碍,探讨亚低温干预的临床效果。方法以2014年1~12月本院108例颅脑外伤患者作为观察组,27例健康志愿者为对照组,测定两组的神经内分泌指标。将观察组随机分为观察A组和观察B组,每组54例,观察A组在常规治疗基础上采取亚低温干预,观察B组采取常规治疗,比较两组治疗后的神经功能缺损评分。结果观察组心钠肽(ANP)、脑钠肽(BNP)浓度明显低于对照组,内源性类洋地黄物质(EDLS)、抗利尿激素(ADH)浓度明显高于对照组,差异有统计学意义(P<0.05);观察组中不同程度脑损伤患者的血ANP、BNP浓度差异无统计学意义(P>0.05),血EDLS、ADH浓度随脑损伤程度加重而增加;治疗30 d后,观察A组神经功能缺损评分为(19.6±4.8)分,观察B组为(27.6±5.4)分,组间比较差异有统计学意义(P<0.05)。结论神经内分泌指标ANP、BNP、EDLS、ADH可作为颅脑外伤的判断性指标,EDLS、ADH对颅脑外伤程度的判断有一定的指导价值,亚低温干预有助于脑外伤患者神经功能的改善。

목적:연구로뇌외상신경내분비적공능장애,탐토아저온간예적림상효과。방법이2014년1~12월본원108례로뇌외상환자작위관찰조,27례건강지원자위대조조,측정량조적신경내분비지표。장관찰조수궤분위관찰A조화관찰B조,매조54례,관찰A조재상규치료기출상채취아저온간예,관찰B조채취상규치료,비교량조치료후적신경공능결손평분。결과관찰조심납태(ANP)、뇌납태(BNP)농도명현저우대조조,내원성류양지황물질(EDLS)、항이뇨격소(ADH)농도명현고우대조조,차이유통계학의의(P<0.05);관찰조중불동정도뇌손상환자적혈ANP、BNP농도차이무통계학의의(P>0.05),혈EDLS、ADH농도수뇌손상정도가중이증가;치료30 d후,관찰A조신경공능결손평분위(19.6±4.8)분,관찰B조위(27.6±5.4)분,조간비교차이유통계학의의(P<0.05)。결론신경내분비지표ANP、BNP、EDLS、ADH가작위로뇌외상적판단성지표,EDLS、ADH대로뇌외상정도적판단유일정적지도개치,아저온간예유조우뇌외상환자신경공능적개선。
Objective To research the nerve endocrine disorders of craniocerebral trauma,and to investigate the clinical effect of hypothermia intervention. Methods 108 cases patients with craniocerebral trauma in our hospital from January to December 2014 were selected as observation group,27 cases healthy volunteers were as control group,neuroendocrine index of two groups was tested.The patients with craniocerebral trauma were randomly divided into observation group A and observation group B,54 cases in each group.Hypothermia was taken to the observation group A on the basis of rou-tine treatment,routine treatment was taken to the observation group B,neural function defect score of two groups after treatment was compared. Results Atrial natriuretic peptide (ANP) and brain natriuretic peptide (BNP) concentration of observation group was lower than that of control group,endogenous digitalis like substance (EDLS) and antidiuretic hor-mone (ADH) concentrations was higher than that of control group,the difference was significant (P<0.05).ANP,BNP con-centration of observation group with different degree of brain injury had no significant difference (P>0.05).EDLS,ADH concentration increased with the severity of brain injury.After 30 days of therapy,nerve function deficit score of obser-vation group A was (19.6±4.8) points,and observation group B was (27.6±5.4) points,the difference was significant be-tween the two groups (P<0.05). Conclusion Neuroendocrine index of ANP,BNP,EDLS,ADH can be used as a judgment index of craniocerebral trauma.EDLS,ADH has certain guiding value on the degree of brain injury,hypothermia inter-vention help to improve nerve function of patients with craniocerebral trauma.
