目的:探讨妊娠期糖尿病孕妇血清趋化素、超敏C-反应蛋白(hs-CRP)与胰岛素抵抗(IR)的关系。方法选择2012年7月~2014年7月在本院妇产科定期产检的孕妇为研究对象。根据血糖筛查结果,随机选取40例新诊断的妊娠期糖尿病患者为1组,40例经饮食、运动控制后血糖控制良好的妊娠期糖尿病孕妇为2组,40例血糖正常组孕妇为3组。采用酶联免疫吸附法检测各组孕妇血清趋化素水平,采用免疫透射比浊法测定检测各组孕妇hs-CRP水平,采用HOMA胰岛素抵抗指数(HOMA-IRI)评价三组孕妇IR程度。比较三组孕妇血清趋化素、hs-CRP及HOMA-IRI,并进行相关性分析。结果1组孕妇hs-CRP、血清趋化素及IR相关指标(FPG、FINS、HOMA-IRI)均显著高于其余两组(P<0.05)。1组血清趋化素、hs-CRP均与IR相关指标呈正相关(P<0.05),3组未发现有相关性(P>0.05)。2组血清趋化素与FINS、HOMA-IRI呈正相关(P<0.05),与FPG无相关性(P>0.05);2组hs-CRP与IR相关指标均呈正相关(P<0.05)。结论妊娠期糖尿病孕妇存在明显的IR,其体内hs-CRP、血清趋化素水平明显升高,在IR的正相关机制中起重要作用。

목적:탐토임신기당뇨병잉부혈청추화소、초민C-반응단백(hs-CRP)여이도소저항(IR)적관계。방법선택2012년7월~2014년7월재본원부산과정기산검적잉부위연구대상。근거혈당사사결과,수궤선취40례신진단적임신기당뇨병환자위1조,40례경음식、운동공제후혈당공제량호적임신기당뇨병잉부위2조,40례혈당정상조잉부위3조。채용매련면역흡부법검측각조잉부혈청추화소수평,채용면역투사비탁법측정검측각조잉부hs-CRP수평,채용HOMA이도소저항지수(HOMA-IRI)평개삼조잉부IR정도。비교삼조잉부혈청추화소、hs-CRP급HOMA-IRI,병진행상관성분석。결과1조잉부hs-CRP、혈청추화소급IR상관지표(FPG、FINS、HOMA-IRI)균현저고우기여량조(P<0.05)。1조혈청추화소、hs-CRP균여IR상관지표정정상관(P<0.05),3조미발현유상관성(P>0.05)。2조혈청추화소여FINS、HOMA-IRI정정상관(P<0.05),여FPG무상관성(P>0.05);2조hs-CRP여IR상관지표균정정상관(P<0.05)。결론임신기당뇨병잉부존재명현적IR,기체내hs-CRP、혈청추화소수평명현승고,재IR적정상관궤제중기중요작용。
Objective To investigate the association of serum chemokine and high-sensitivity C-reactive protein (hs-CRP)with insulin resistance(IR)in gestational diabetes mellitus (GDW). Methods The pregnant women examined regu-larly in the department of gynecology and obstetrics in our hospital from July 2012 to July 2014 were selected as study objects.According to the results of blood glucose screening,40 cases of the newly diagnosed pregnant women with dia-betes were randomly selected as group one,40 cases of pregnant women with GDM whose blood glucose was well con-trolled by controlling their diet and motion were randomly selected as group two and another 40 cases of pregnant women with normal glucose tolerance were selected as group three.The serum chemokine level in the pregnant women of each group were checked by using enzyme-linked immunosorbent assay.Determination of serum hs-CRP level of the pregnant women in each group was detected by immune transmission turbidimetry.The IR degree of the pregnant women in each group was valued by using HOMA insulin resistance index (HOMA-IRI).The serum chemokine,hs-CRP and HOMA-IR of the pregnant women among three groups were compared and analyzed. Results The correlation indexes of serum hs-CRP,chemokine and IR (FPG,FINS,HOMA-IRI)of the pregnant women in group one was significantly higher than that in other two groups respectively (P<0.05).The serum chemokine,hs-CRP related with IR index in group one were positively correlated (P<0.05),but in group three they were not related (P>0.05).The serum chemokine related with FINS and HOMA-IRI in group two was positively (P<0.05),and had no correlation with FPG (P>0.05).The hs-CRP and IR related indexes were positively correlated in group two(P<0.05). Conclusion The IR obviously exists in the pregnant women with GDM and the hs-CRP and serum chemokine in their body obviously increase.The mechanism of IR in pos-itive correlation plays an important role.
