目的：探讨老年胃癌患者术后并发症的相关因素。方法回顾性地分析中国石油乌鲁木齐石油化工总厂职工医院外一科2004年1月～2014年12月接受手术治疗并经病理确证为胃癌的老年患者57例，分析术后并发症情况及老年胃癌患者术后并发症的危险因素。结果本组57例行胃癌手术患者中，并发症发生18例，并发症发生率为31．6％，其中切口感染3例（5．3％），吻合口瘘2例（3．5％），切口开裂2例（3．5％），肺部感染3例（5．3％），十二指肠残端瘘1例（1．8％）。炎性肠梗阻3例，残胃无力1例，腹腔感染2例，吻合口出血1例。经分析显示，术前合并症、肿瘤直径、发病年龄及手术时间与老年胃癌患者术后并发症发生有明显关系（P＜0．05）。结论老年胃癌患者因年龄和疾病因素，术后并发症也随之增加，基层医院应重视老年胃癌患者围手术期的观察和处理。
목적：탐토노년위암환자술후병발증적상관인소。방법회고성지분석중국석유오로목제석유화공총엄직공의원외일과2004년1월～2014년12월접수수술치료병경병리학증위위암적노년환자57례，분석술후병발증정황급노년위암환자술후병발증적위험인소。결과본조57례행위암수술환자중，병발증발생18례，병발증발생솔위31．6％，기중절구감염3례（5．3％），문합구루2례（3．5％），절구개렬2례（3．5％），폐부감염3례（5．3％），십이지장잔단루1례（1．8％）。염성장경조3례，잔위무력1례，복강감염2례，문합구출혈1례。경분석현시，술전합병증、종류직경、발병년령급수술시간여노년위암환자술후병발증발생유명현관계（P＜0．05）。결론노년위암환자인년령화질병인소，술후병발증야수지증가，기층의원응중시노년위암환자위수술기적관찰화처리。
Objective To investigate the related factors associated with postoperative complications in elderly patients with gastric cancer. Methods A retrospective data of 57 elderly patients with gastric cancer between Jan 2004 and Dec 2014 were studied. Postoperative complications and risk factors were analyzed. Results There were 18 cases of postoperative complications in all 57 patients (31%),including wound infection in 3 cases (5.3%), anastomotic leakage 2 cases (3.5%), abdominal wound disruption 2 cases (3.5%), duodenal stump fistula one case, early postoperative inflammatory intestinal obstruction 3 cases, intra-abdominal infection 2 cases, functional gastric atony one case, anastomotic bleeding one case, lung infection 3 cases. Preoperative comorbidities , tumor size , age and operative time were correlated with postoperative complications of gastric cancer (P<0.05). Conclusion Age and disease factors were influence factors of postoperative complications; elderly patients should pay attention to the observation and treatment of gastric surgery around the stage.
