目的：比较磁共振(MRI)与临床检查在前交叉韧带(ACL)损伤中的诊断价值。方法选取本院2012年3月~2014年3月收治的80例急性创伤性膝关节血肿患者作为研究对象，入院后均行临床检查和MRI检查，以关节镜检查作为金标准，观察两种方式对前交叉韧带损伤的诊断情况，并比较其诊断准确度、敏感度和特异度。结果 MRI检查的真阳性率为68.75%，显著高于临床检查的40.00%，差异有统计学意义(P<0.05)。 MRI检查的假阴性率为2.50%，显著低于临床检查的23.75%，差异有统计学意义(P<0.05)。 MRI检查的准确度和敏感度显著高于临床检查，差异有统计学意义(P<0.05)。结论 MRI具有准确度、敏感度高的特点，在ACL损伤早期诊断中具有较高应用价值。
목적：비교자공진(MRI)여림상검사재전교차인대(ACL)손상중적진단개치。방법선취본원2012년3월~2014년3월수치적80례급성창상성슬관절혈종환자작위연구대상，입원후균행림상검사화MRI검사，이관절경검사작위금표준，관찰량충방식대전교차인대손상적진단정황，병비교기진단준학도、민감도화특이도。결과 MRI검사적진양성솔위68.75%，현저고우림상검사적40.00%，차이유통계학의의(P<0.05)。 MRI검사적가음성솔위2.50%，현저저우림상검사적23.75%，차이유통계학의의(P<0.05)。 MRI검사적준학도화민감도현저고우림상검사，차이유통계학의의(P<0.05)。결론 MRI구유준학도、민감도고적특점，재ACL손상조기진단중구유교고응용개치。
Objective To compare the diagnosis value of magnetic resonance imaging(MRI) and clinical examination in anterior cruciate ligament (ACL) injury. Methods 80 patients of acute traumatic knee joint hematoma from March 2012 to March 2014 in our hospital were selected as the research object.All patients were underwent clinical examination and MRI after admission.With arthroscopy as the gold standard for diagnosis,the diagnosis of two kinds of methods on anterior cruciate ligament injury was observed,and the diagnostic accuracy,sensitivity and specificity was compared. Results The true positive rate of MRI examination was 68.75%,which was higher than 40% of clinical examination,with significant difference (P<0.05).The false negative rate of MRI examination was 2.50%,which was lower than 23.75% of the clinical examination,with significant difference (P<0.05).The accuracy and the sensitivity of MRI examination was higher than that of clinical examination,with significant difference (P<0.05). Conclusion MRI has the characteristics of high accuracy,high sensitivity,and has higher application value in early diagnosis of ACL injury.
