目的:探讨双管喉罩用于COPD呼吸衰竭无创通气中的应用效果。方法选取本院2012年1月~2014年6月收治的60例COPD合并Ⅱ型吸吸衰竭患者作为研究对象,随机分为两组,各30例。观察组给予双管喉罩,对照组给予普通口鼻面罩,比较两组的SpO2上升时间、动脉血气改善时间、通气失败率、脱机时间、并发症发生率。结果观察组的SpO2上升时间、脱机时间显著短于对照组,差异有统计学意义(P<0.01)。观察组的反流误吸率、通气失败率显著低于对照组,差异有统计学意义(P<0.05)。观察组治疗后8、48 h的pH、PaCO2水平显著高于对照组,观察组的PaCO2水平显著低于对照组,差异有统计学意义(P<0.01)。结论双管喉罩可以更好地改善COPD合并Ⅱ型呼吸衰竭患者的预后,能够替代口鼻面罩用于该类患者的无创通气治疗。

목적:탐토쌍관후조용우COPD호흡쇠갈무창통기중적응용효과。방법선취본원2012년1월~2014년6월수치적60례COPD합병Ⅱ형흡흡쇠갈환자작위연구대상,수궤분위량조,각30례。관찰조급여쌍관후조,대조조급여보통구비면조,비교량조적SpO2상승시간、동맥혈기개선시간、통기실패솔、탈궤시간、병발증발생솔。결과관찰조적SpO2상승시간、탈궤시간현저단우대조조,차이유통계학의의(P<0.01)。관찰조적반류오흡솔、통기실패솔현저저우대조조,차이유통계학의의(P<0.05)。관찰조치료후8、48 h적pH、PaCO2수평현저고우대조조,관찰조적PaCO2수평현저저우대조조,차이유통계학의의(P<0.01)。결론쌍관후조가이경호지개선COPD합병Ⅱ형호흡쇠갈환자적예후,능구체대구비면조용우해류환자적무창통기치료。
Objective To explore the application effect of double tube laryngeal mask in noninvasive ventilation on chronic obstructive pulmonary disease(COPD) patient with respiratory failure. Methods 60 patients with COPD and typeII respiratory failure from January 2012 to June 2014 in our hospital were selected and randomly divided into two groups,30 cases in each group.The observation group was given the double tube laryngeal mask,the control group was given the ordinary nose and mouth mask.The SpO2 rise time,arterial blood gas improvement time,ventilation failure rate, the offline time,the incidence rate of complication in two groups was compared. Results The SpO2 rise time,the offline time of the observation group was shorter than that of the control group,with significant difference(P<0.01).The regurgi-tation and aspiration rate,ventilation failure rate in the observation group was lower than that in the control group,with significant difference(P<0.05).The level of pH and PaCO2 in the observation group at 8,48 h after treatment was higher than that in the control group,the level of PaCO2 in the observation group was lower than that in the control group,with significant difference (P<0.01). Conclusion Double tube laryngeal mask can improve the prognosis in patients with COPD and type II respiratory failure and replace nose and mouth mask for noninvasive ventilation treatment on these patients.
