目的：探讨内镜下钛夹联合注射肾上腺治疗非静脉曲张性上消化道出血疗效。方法笔者所在医院经胃镜检查确诊为非静脉曲张性上消化道出血的患者120例，随机分为钛夹治疗组，肾上腺素药物治疗组及钛夹联合注射肾上腺治疗组，各40例，对比分析以上3种内镜下治疗方法的止血率及再出血率的差异。结果3组即时止血率均为100％；钛夹组有7例发生再次出血，药物组有13例发生再出血，注射联合钛夹组中有1例发生再出血，再出血率分别为17．5％，32．5％，2．5％，三组差异有统计学意义（P＜0．05）。结论胃镜下注射肾上腺素联合钛夹治疗非静脉曲张性上消化道出血疗效可靠，优于单纯钛夹治疗。
목적：탐토내경하태협연합주사신상선치료비정맥곡장성상소화도출혈료효。방법필자소재의원경위경검사학진위비정맥곡장성상소화도출혈적환자120례，수궤분위태협치료조，신상선소약물치료조급태협연합주사신상선치료조，각40례，대비분석이상3충내경하치료방법적지혈솔급재출혈솔적차이。결과3조즉시지혈솔균위100％；태협조유7례발생재차출혈，약물조유13례발생재출혈，주사연합태협조중유1례발생재출혈，재출혈솔분별위17．5％，32．5％，2．5％，삼조차이유통계학의의（P＜0．05）。결론위경하주사신상선소연합태협치료비정맥곡장성상소화도출혈료효가고，우우단순태협치료。
Objective To study the endoscopic titanium clips combined injection of adrenal for hemorrhage of non variceal upper gastrointestinal tract. Methods In author's hospital 120 cases diagnosed by gastroscopy as non variceal upper digestive tract hemorrhage were randomly divided into the treatment groups of titanium clip,of adrenergic drug and of titanium clips combined injecting adrenal,40 cases in each group;the differences among above three kinds of endoscopic therapy in hemostatic rate and the rate of rebleeding were analyzed. Results In three groups the immediate hemostasis rate all was 100%;in titanium clip group 7 cases had rebleeding,13 cases of rebleeding occurred in the drug group,in injection combined with titanium clip group 1 case had rebleeding, thus rebleeding rate were 17.5%,32.5%,2.5%,respectively;there was significant difference between three groups (P<0.05). Conclusion Endoscopic epinephrine injection combined with titanium clips in the treatment of non variceal upper gastrointestinal hemorrhage has reliable curative effect,and it's better than that of alone titanium clamp.
