目的:观察谐振效应引起椎间盘源性下腰痛相关炎性指标血清含量的动态变化,探讨谐振对椎间盘的损伤机制和椎间盘源性下腰痛的早期血清生物标志物。方法取90只成年健康的日本大耳白兔,随机分为5组,每组18只。A组为正常对照组;B为造模对照组;C为4 Hz振动组;D为5 Hz振动组;E为6 Hz振动组。分别于2、4、6周每组在清晨空腹下经耳源静脉穿刺抽取5 ml静脉血,装于抗凝试管,离心取上清液在低温下保存,分别用ELISA法进行6-酮前列腺素1α(6-keto-PGF 1α)、白细胞介素1、肿瘤坏死因子-α的血清含量测定,观察其动态变化。结果振动刺激6 Hz组及5 Hz组血清6-keto-PGF 1α的浓度在第2周末,第4周末、第6周末均明显高于造模对照组(P<0.05),振动刺激6 Hz组血清6-keto-PGF 1α浓度升高时间明显早于血清IL-1β浓度和血清TNF-α浓度。结论谐振与椎间盘源性下腰痛的炎性疼痛机制密切相关,可能导致椎间盘组织通透性增大并产生的高浓度炎性因子;监测血清6-keto-PGF 1α浓度可以较早、稳定地反映椎间盘源性下腰痛内炎性介质的变化,为椎间盘源性下腰痛的早期诊断提供了一个很好的参考。

목적:관찰해진효응인기추간반원성하요통상관염성지표혈청함량적동태변화,탐토해진대추간반적손상궤제화추간반원성하요통적조기혈청생물표지물。방법취90지성년건강적일본대이백토,수궤분위5조,매조18지。A조위정상대조조;B위조모대조조;C위4 Hz진동조;D위5 Hz진동조;E위6 Hz진동조。분별우2、4、6주매조재청신공복하경이원정맥천자추취5 ml정맥혈,장우항응시관,리심취상청액재저온하보존,분별용ELISA법진행6-동전렬선소1α(6-keto-PGF 1α)、백세포개소1、종류배사인자-α적혈청함량측정,관찰기동태변화。결과진동자격6 Hz조급5 Hz조혈청6-keto-PGF 1α적농도재제2주말,제4주말、제6주말균명현고우조모대조조(P<0.05),진동자격6 Hz조혈청6-keto-PGF 1α농도승고시간명현조우혈청IL-1β농도화혈청TNF-α농도。결론해진여추간반원성하요통적염성동통궤제밀절상관,가능도치추간반조직통투성증대병산생적고농도염성인자;감측혈청6-keto-PGF 1α농도가이교조、은정지반영추간반원성하요통내염성개질적변화,위추간반원성하요통적조기진단제공료일개흔호적삼고。
Objective To observe the inflammatory factors change of serum in discogenic low back pain related to inflammatory marker by resonance frequency,to explore the disc-injuried mechanisms and early serum markers,in order to provide an important theoretical basis for the prevention and early diagnosis of discogenic low back pain caused by vibration. Methods Ninety healthy rabbits were randomly divided into 5 groups (n=18). Groups A as normal group;Groups B as control group;Groups C as 4 Hz vibration group;Groups D as 5 Hz vibration group;Groups E as 6 Hz vibration group. Experimental interventions and processes were as group A:free;Group B: to imitate driver forced upright buttocks seat,no vibration,1h/d. The other three groups mimic the driver's buttocks to sit at a special fixed-cage vibration stage at different frequencies (2,3,4 Hz),vibration, vibration time 1h/d. In the second,fourth,sixth weekend morning,for each group 5ml fasting venous blood by ear vein puncture was extracted,installed in the anticoagulant test tube,the centrifuged supernatant was stored at low temperatures,respectively progress 6-keto-PGF 1α,IL-1β,TNF-α serum content were determined by using ELISA,and so to find their dynamic changes. Results The 6-keto-of PGF 1α content of 6 Hz vibration group and 5 Hz vibration group were significantly higher than that of the control group in the second ,fourth and sixth weekend (P<0.05),and the serum 6-keto-of PGF 1α content change of 6 Hz vibration group was significantly earlier than serum IL-1β content and serum TNF-α concentration. Conclusion Inflammatory pain mechanisms of discogenic low back pain is closely related to vibration,and might lead to disc tissue permeability increased and high content of inflammatory factors occwred;monitoring of serum 6-keto-of PGF 1α content can earlier and stably show changes of inflammatory factors in the discogenic low back pain,thus provide a good reference for early diagnosing discogenic low back pain.
