目的:研究分析产前超声检查顺序对脐动脉(UmA)和大脑动脉(MCA)血流的动力学影响。方法:随机选取2014年1月至8月来我院行产前超声检查的80例孕妇作为研究对象,均行超声检查,比较并分析产前超声检查的前后的血流动力学变化。结果:经超声检查后的大脑中动脉的阻力指数(RI)、波动指数(PI)都有明显的下降[PI (1.72±0.21) VS (1.60±025),t=3.366,P<0.001;RI (0.80±0.06) VS (0.78±0.05),t=3.598,P<0.001],在舒张的末期血流速度要明显的升高[EDV (7.81±3.79) VS (8.90±5.17),t=2.22,P<0.05];MCA出现舒张期的血流缺失的概率要明显增多(曾2=4.55,P<0.05)。对其进行产前的检查后脐动脉的波动指数出现明显的下降[PI (1.02±0.26) VS (0.96±0.21),t=2.08,P<0.05],但大脑中动脉其他的血流动力学和血流的频谱形态没有明显的改变。结论:在产前使用系统超声检查可能会对胎儿的脐动脉和大脑中动脉的血流动力学产生一定的影响,所以建议把胎儿血流动力学的超声检测放在所有超声检查之前。

Objective:to study the order of analysis of prenatal ultrasonography for umbilical artery (UmA) and cerebral arter-ies ( MCA) blood flow dynamics of impact. Methods: from January 2014 to August came to my hospital and prenatal ultra-sonography of 80 pregnant women as research subjects underwent ultrasound examination, comparison and analysis of prenatal ultrasonography of the hemodynamic changes before and after. Results: after ultrasound examination of the Middle cerebral artery resistance index (RI) , the volatility index (PI) are down dramatically [PI (1. 72± 0. 21) VS (1. 60± 025) , t=3. 366,P<0. 001;RI (0. 80±0. 06) VS (0. 78±0. 05) , t=3. 598,P<0. 001] , late diastolic velocity of blood flow increased sig-nificantly [EDV (7. 81 ± 3. 79) VS (8. 90 ± 5. 17) , t=2. 22,P<0. 05] ; MCA diastolic occurred significantly increases the probability of loss of blood flow (x2=4. 55,P<0. 05) . Prenatal checks for its umbilical arteries after the clear decline in volatility index [PI (1. 02±0. 26) VS (0. 96±0. 21) , t=2. 08,P<0. 05] , but elsewhere in the brain artery hemodynamics and blood spectrum morphology has not changed significantly. Conclusion: the prenatal ultrasound examination of the system could have on fetus umbilical artery and middle cerebral artery hemodynamics had effects, so recommend ultrasound detection of fetal hemodynamics in all before ultrasound.
