目的:探析BiPAP呼吸机治疗重叠综合征患者对其血管内皮功能及心功能的作用机制。方法选取2013年7月至2014年7月我院收治的94例重叠综合征患者为研究对象,按治疗方法的不同分为实验组和对照组各47例,对照组实施常规疗法,实验组在常规疗法的基础上实施BiPAP无创呼吸机疗法。比较两组患者的临床总有效率、心功能以及血管内皮功能变化情况。结果实验组的临床总有效率为85.1%,显著高于对照组的59.6%,差异具有统计学意义(P<0.05);两组患者治疗后的脑钠肽水平、左心室Tei指数、右心室Tei指数等心功能指标以及血管内皮素-1、一氧化氮、凝血酶-Ⅲ等血管内皮功能指标均有改善,且实验组改善程度显著优于对照组,差异具有统计学意义(P<0.05)。结论采用BiPAP无创呼吸机治疗重叠综合征患者,对纠正二氧化碳潴留及缺氧情况有效,可显著改善患者的血管内皮功能及心功能,临床效果确切,值得推广。

목적:탐석BiPAP호흡궤치료중첩종합정환자대기혈관내피공능급심공능적작용궤제。방법선취2013년7월지2014년7월아원수치적94례중첩종합정환자위연구대상,안치료방법적불동분위실험조화대조조각47례,대조조실시상규요법,실험조재상규요법적기출상실시BiPAP무창호흡궤요법。비교량조환자적림상총유효솔、심공능이급혈관내피공능변화정황。결과실험조적림상총유효솔위85.1%,현저고우대조조적59.6%,차이구유통계학의의(P<0.05);량조환자치료후적뇌납태수평、좌심실Tei지수、우심실Tei지수등심공능지표이급혈관내피소-1、일양화담、응혈매-Ⅲ등혈관내피공능지표균유개선,차실험조개선정도현저우우대조조,차이구유통계학의의(P<0.05)。결론채용BiPAP무창호흡궤치료중첩종합정환자,대규정이양화탄저류급결양정황유효,가현저개선환자적혈관내피공능급심공능,림상효과학절,치득추엄。
Objective To study the action mechanism of BiPAP ventilator on cardiac function and vascular endothelial function of patients with overlap syndrome (OS). Methods 94 cases of OS patients admitted to our hospital from July 2013 to July 2014 were selected as objects and divided into two groups according to different treatment methods, with 47 cases in each group. The control group received routine therapy, while the experimental group was implemented BiPAP noninvasive ventilator treatment on the basis of the routine therapy. The clinical effect, changes of cardiac function and vascular endothelial function between two groups were compared. Results The total effective rate of experimental group was 85.1%, significantly higher than 59.6%of control group, with statistical difference (P<0.05);the cardiac function indicators (including BNP, Tei-index of left and right ventricle) and vascular endothelial function indicators (including vascular endothelin-1, NO, thrombin-Ⅲ) were improved, and the improvement degree of experimental group was better than that of control group, with statistical difference (P <0.05). Conclusions Noninvasive BiPAP ventilator has effect on the correction of carbon dioxide retention and oxygen deficit in treating patients with overlap syndrome, and can significantly improve patients' vascular endothelial function and cardiac function, with definite effect, which deserves promotion.
