目的：统计并分析人工全髋关节置换（ THR）围手术期失血与输血量及输血率的大小。方法2003年至2012年于本组行THR的患者共718例，男314例，女404例；平均年龄56．1岁（17～87岁）。统计患者性别、年龄、体重、身高、BMI指数、术后引流量、术后输血量、术前HGB、HCT，术后最低和出院前末次HGB、HCT以及出现时间，术中出血以及异体及自体血回输情况。结果术中出血平均为（500．6±395．1）ml，术中输血率为26．7％，平均用血量为0．08U RBC。术后引流平均为（457．6±397．6）ml，术后输血率39％，平均用血量为1．41U RBC，围手术期总体输血率为50．8％，平均用血量为2．22 U RBC。年龄、BMI指数、术前HGB浓度、术中出血、术后引流量是影响患者围手术期输血的主要相关因素。结论 THR围手术期出血较多，输血率较高，注意围手术期血液管理，对减少输血及降低输血风险有积极的意义。
목적：통계병분석인공전관관절치환（ THR）위수술기실혈여수혈량급수혈솔적대소。방법2003년지2012년우본조행THR적환자공718례，남314례，녀404례；평균년령56．1세（17～87세）。통계환자성별、년령、체중、신고、BMI지수、술후인류량、술후수혈량、술전HGB、HCT，술후최저화출원전말차HGB、HCT이급출현시간，술중출혈이급이체급자체혈회수정황。결과술중출혈평균위（500．6±395．1）ml，술중수혈솔위26．7％，평균용혈량위0．08U RBC。술후인류평균위（457．6±397．6）ml，술후수혈솔39％，평균용혈량위1．41U RBC，위수술기총체수혈솔위50．8％，평균용혈량위2．22 U RBC。년령、BMI지수、술전HGB농도、술중출혈、술후인류량시영향환자위수술기수혈적주요상관인소。결론 THR위수술기출혈교다，수혈솔교고，주의위수술기혈액관리，대감소수혈급강저수혈풍험유적겁적의의。
Objective To explore the loss of blood and transfusion volume and rate in total hip replacement.Methods From 2003 to 2012,718 patients who underwent total hip replacement surgery were selected according to the inclusion criteria .The enrolled patients were composed of 314 males and 404 females (age ranging from 17 to 87 years, 56.1 years in average).Sex、age、weight、height、BMI、drainage、transfusion volume、pre-and post-operation HGB、HCT were analyzed .Results The average blood loss during the operation was 500.6 ±395.12ml, the transfusion rate was 26.7%, and the average volume was 0.08U RBC.The average drainage after the operation was 457.6 ±397.55ml, the transfusion rate was 39%, the average volume was 1.41U RBC; the total transfusion rate of the perioperative period was 50. 8%, and the average volume was 2.22U RBC.Conclusion The perioperative blood loss of total hip replacement is in a large quantity , and the transfusion rate is high; the main relative factors include age , BMI, HGB before operation, blood loss during operation、and drainage.
