目的:观察不同剂量顺式阿曲库铵对丙泊酚浅麻醉状态下听觉诱发电位指数(AAI)的影响。方法将择期手术全麻患者80例随机分为对照组(C组)、2ED95(E2组)、4ED95(E4组)及8ED95(E8组)顺式阿曲库铵组4组,分别于诱导后将丙泊酚效应室靶浓度设为2.5μg/m L ,稳定后C组静脉注射生理盐水5 m L ;E2、E4、E8组分别静脉注射2ED95(0.1 mg/kg),4ED95(0.2 mg/kg),8ED95(0.4 mg/kg)顺式阿曲库铵。注药后观察15 min停止试验,记录注药前5 min(T0)、注药后第1个5 min(T1)、第2个5 min(T2)及第3个5 min(T3)内各实测听觉诱发电位指数(AAI)、肌电图(EMG)平均值。结果与T0相比,E2、E4、E8各组 AAI值在T1、T2、T3时段内均降低,差别有统计学意义(P<0.05);与C组比较,E2、E4、E8组注药后各时段 AAI值均显著降低(P<0.05)。EMG在E2、E4、E8组T1、T2、T3时段内与T0相比均显著降低(P<0.001)。结论浅麻醉状态下,静脉注射顺式阿曲库铵可使AAI值显著下降,下降的时段与肌松药的药代动力学相关;不同剂量间下降幅度差别无统计学意义。

목적:관찰불동제량순식아곡고안대병박분천마취상태하은각유발전위지수(AAI)적영향。방법장택기수술전마환자80례수궤분위대조조(C조)、2ED95(E2조)、4ED95(E4조)급8ED95(E8조)순식아곡고안조4조,분별우유도후장병박분효응실파농도설위2.5μg/m L ,은정후C조정맥주사생리염수5 m L ;E2、E4、E8조분별정맥주사2ED95(0.1 mg/kg),4ED95(0.2 mg/kg),8ED95(0.4 mg/kg)순식아곡고안。주약후관찰15 min정지시험,기록주약전5 min(T0)、주약후제1개5 min(T1)、제2개5 min(T2)급제3개5 min(T3)내각실측은각유발전위지수(AAI)、기전도(EMG)평균치。결과여T0상비,E2、E4、E8각조 AAI치재T1、T2、T3시단내균강저,차별유통계학의의(P<0.05);여C조비교,E2、E4、E8조주약후각시단 AAI치균현저강저(P<0.05)。EMG재E2、E4、E8조T1、T2、T3시단내여T0상비균현저강저(P<0.001)。결론천마취상태하,정맥주사순식아곡고안가사AAI치현저하강,하강적시단여기송약적약대동역학상관;불동제량간하강폭도차별무통계학의의。
Objective To observe the effect of different doses of cisatracurium on auditory evoked potential index undergoing light anesthesia . Methods 80 patients undergoing elective operation with general anesthesia were randomly assigned to control group (group C) ,2ED95 (group E2) ,4ED95 (group E4) ,8ED95 (group E8) ,20 cases each group . The cisatracurium concentrations for group E2 ,group E4 ,and group E8 were 0 .1 mg/kg ,0 .2 mg/kg ,and 0 .4 mg/kg respectively . 5 mL of saline or cisatra‐curium injection was administered while the effect target concentration of propofol was set at 2 .5 μg/mL . The patients were observed for 15 min before the test ended . Average value of AAI and EMG were re‐corded at time of before injection (T0 ) ,and first 5 min(T1 ) ,second 5 min(T2 ) ,third 5 min(T3 ) after in‐jection . Results Compared with baseline(T0 ) ,values of AAI in T1‐3 period were decreased in cisatracuri‐um‐treated groups(P< 0 .05) . Compared with control group ,value of AAI in cisatracurium‐treated groups in T1‐3 period was decreased(P<0 .05) . Compared with T0 ,values of EMG in T1‐3 period were de‐creased in cisatracurium‐treated groups(P<0 .001) . Conclusion Value of AAI can be decreased ,with a statistical significance ,after cisatracurium injection undergoing light general anesthesia . The difference in AAI decrease among different doses of cisatracurium was not statistically significant though .
