妇科恶性肿瘤发病率逐年上升,其中卵巢癌死亡率占5%,居妇科恶性肿瘤首位。治疗首选切除双侧卵巢,其术后骨质疏松发生率高,严重影响中老年女性生活质量。常用激素替代治疗(HRT)和植物雌激素(phyto-oestrogens,PE)治疗绝经后骨质疏松症,激素替代治疗虽然降低了骨质疏松和骨折的危险性,但增加了乳腺癌、子宫内膜癌等发病率。而大多数妇科恶性肿瘤是激素依赖型肿瘤,PE化学结构与17β-雌二醇相似,具有弱雌二醇作用,应用于妇科恶性肿瘤术后骨质疏松治疗安全性有待考证。橄榄苦苷是从天然橄榄油中提取的主要成分,不仅可以减少骨量丢失防治骨质疏松,还具有抗子宫内膜癌、抗乳腺癌等作用,已经成了较理想的治疗妇科恶性肿瘤术后骨质疏松的候选药。本文对近几年其治疗骨质疏松的可能机制做一综述。

The incidence of gynecological malignant tumors is increasing.The mortality of the ovarian cancer accounts for 5%, which ranks first in gynecological malignant tumor.Bilateral ovariectomy is the first choice of the treatment.However, the incidence of postoperative osteoporosis is high, seriously affecting the quality of life in elderly women.Hormone replacement therapy ( HRT) and phytoestrogen ( phytoestrogen PE) are commonly used in the treatment of postmenopausal osteoporosis.HRT can reduce the risk of osteoporosis and fractures, but increase the incidence of breast cancer and endometrial cancer.Most gynecological malignant tumors are hormone-dependent tumors.The chemical structure of PE is similar to that of 17β-estradiol.PE has weak estrogen-like effect.Its safety in the treatment of osteoporosis needs to be proven.Oleuropein is the major component extracted from natural olive oil.It can not only reduce the loss of bone mass in prevention and treatment of osteoporosis, but also has treatment effect in endometrial cancer and breast cancer.It has become a candidate drug for postoperative osteoporosis after the treatment of gynecological malignant tumors.This paper reviews the mechanism of osteoporosis based on the recent findings.
