目的：通过对老年人老年护理服务现状的调查，分析老年护理服务需求的影响因素，因地制宜地提出具有可行性的老年护理服务发展对策，以提高老年人生命质量。方法2014年3—7月，对江苏省镇江市主管老年护理服务工作的卫生局和民政局相关部门进行调研；同时，对镇江市区部分社区居民进行老年护理服务需求问卷调查，并分析影响老年护理服务需求的影响因素。结果共发放问卷500份，回收有效问卷478份，有效回收率为95.6%。不同年龄、受教育程度、职业、月收入、生活自理能力的老年人老年护理服务需求比较，差异有统计学意义（ P <0.05）。41.4%（198/478）老年人有老年护理服务需求，58.6%（280/478）老年人暂时尚无老年护理服务需求。82.3%（163/198）老年人希望有老年保健服务，76.8%（152/198）老年人希望有住养护理服务，56.1%（111/198）老年人希望有康复训练服务。54.5%（108/198）老年人能接受的最高收费标准是<50元·床-1·d -1，38.4%（76/198）能接受的最高收费标准是50~100元·床-1·d -1。相关性分析结果显示，年龄与生活自理能力呈负相关（ r=-0.305，P <0.01）；月收入与受教育程度呈正相关（r =0.420，P <0.01）。多因素 Logistic 回归分析结果显示，月收入、生活自理能力、新型农村合作医疗保险是老年护理服务需求的影响因素（P <0.05）。结论月收入越高、生活自理能力越差老年人老年护理服务需求度越高，享受新型农村合作医疗保险老年人老年护理服务需求低。社会与政府应当重视老年护理，加强机构建设，提供多样化的服务，重视人才培养，发展老年护理保险，增强管理水平，明确服务标准。

Objectjve To investigate the present situation of nursing care service for old people,to analyse the influencing factors of nursing care service demand for old people,to provide practical countermeasures for nursing care service development for old people in line with local conditions,thus the quality of life of old people could be promoted. Methods From March to July 2014,the study interviewed the managers of public health bureau and civil affairs bureau who took charge of nursing care service for old people in Zhenjiang. At the same time,the study carried on the questionnaire survey among Zhenjiang residents to understand nursing care service for old people,and analyzed the influencing factors of nursing care service demand for old people. Results A total of 500 questionnaires were distributed,and 478 eligible questionnaires were collected,thus the valid return rate was 95. 6% . There were significant differences in the nursing care demand extent for old people among groups with different age,education level,occupation,monthly income,self - care ability and medical insurance( P < 0. 05 ). 41. 4%(198 / 478)of old people had nursing care service demand,and 58. 6%(280 / 478)of old people did not have nursing care service demand for the time being. 82. 3%(163 / 198)of old people had health care service demand,76. 8%(152 / 198) needed hospitalization nursing care service,56. 1%(111 / 198)needed rehabilitation training service. 54. 5%(108 / 198)of old people could afford no more than 50 Yuan per bed per day,38. 4% (76 / 198)could afford 50 - 100 Yuan per bed per day. According to correlation analysis results,age was correlated negatively with self - care ability(r = - 0. 305,P < 0. 01), monthly income was correlated positively with education level(r = 0. 420,P < 0. 01). According to multiple Logistic regression analysis results,monthly income,self - care ability and new rural cooperative medical insurance were influencing factors of nursing care service demand for old people(P < 0. 05). Conclusjon Old people with higher monthly income and poorer self -care ability have higher nursing care demand extent,while old people with new rural cooperative medical insurance have lower nursing care demand extent. The government and society should pay attention to nursing care service for old people and personnel training,strengthen institution construction,provide diversified services for the old,develop geriatric nursing insurance, improve the management level,clarify the service standards.
