目的:研究蛋白磷酸酶2A(PP2a)在胃癌及癌旁正常组织中的表达及幽门螺杆菌(H.pylori)感染与胃癌组织中PP2a表达的相关性。方法入组40例胃癌患者的癌组织(胃癌组)及癌旁组织(癌旁组),2组采用免疫组织化学法检测PP2a蛋白表达情况,TUNEL法检查细胞凋亡,碳14尿素酶呼吸试验检测40例胃癌患者H.pylori感染情况。结果在胃癌组织中PP2a的阳性表达率为70%,明显低于正常胃黏膜组织中PP2a阳性表达率的90%,二者差异有统计学意义(P<0.05);PP2a蛋白表达与胃癌的分化程度、有无淋巴结转移有关;PP2a表达阴性组的胃癌细胞凋亡指数显著低于PP2a阳性组(P<0.05)。胃癌患者H.pylori感染率为57.5%,H.pylori阳性胃癌组织的PP2a蛋白阳性表达率显著低于H.pylori阴性者(P<0.05)。H.pylori感染与胃癌组织中PP2a的表达呈负相关。结论 PP2a可能参与到胃癌的发生发展过程,而H.pylori感染可能通过降低胃黏膜肿瘤抑制基因PP2a蛋白表达水平引起胃黏膜癌变。

목적:연구단백린산매2A(PP2a)재위암급암방정상조직중적표체급유문라간균(H.pylori)감염여위암조직중PP2a표체적상관성。방법입조40례위암환자적암조직(위암조)급암방조직(암방조),2조채용면역조직화학법검측PP2a단백표체정황,TUNEL법검사세포조망,탄14뇨소매호흡시험검측40례위암환자H.pylori감염정황。결과재위암조직중PP2a적양성표체솔위70%,명현저우정상위점막조직중PP2a양성표체솔적90%,이자차이유통계학의의(P<0.05);PP2a단백표체여위암적분화정도、유무림파결전이유관;PP2a표체음성조적위암세포조망지수현저저우PP2a양성조(P<0.05)。위암환자H.pylori감염솔위57.5%,H.pylori양성위암조직적PP2a단백양성표체솔현저저우H.pylori음성자(P<0.05)。H.pylori감염여위암조직중PP2a적표체정부상관。결론 PP2a가능삼여도위암적발생발전과정,이H.pylori감염가능통과강저위점막종류억제기인PP2a단백표체수평인기위점막암변。
Objective To study the expression of PP2a in gastric carcinoma and adjacent normal tissues, and reveal the correlation between PP2a in gastric carcinoma tissues and Helicobacter pylori infection. Methods PP2a protein in 40 gastric carcinoma tissues and 40 paracancerous tissues were detected by immunohistochemical technique. The cell apoptosis was detected by TUNEL. H.pylori was identified by 14 carbon and urease breath test (14C-UBT) method. Results The positive expression of PP2a protein in gastric carcinoma tissues was 70%, significantly lower than that of paracancerous tissues of 90%(P<0.05);The rates of PP2a protein expression was associated with the tumor differentiation, lymph node metastasis (P<0.05). The apoptosis index in PP2a negative expression group was much lower than that in the positive group (P<0.05). The positive expression of PP2a in H.pylori positive gastric cancer tissue was significantly lower than the H.pylori negative (P<0.05). H.pylori infection was negatively correlated to the expression of PP2a protein in gastric carcinoma tissues (r=-0.342, P<0.05). Conclusion PP2a was closely associated with the occurrence and the development of gastric cancer. The H.pylori may cause gastric carcinogenesis by reducing PP2a protein expression levels.
