在ICU内，脓毒症及感染性休克是引起急性肾损伤（ AKI）非常重要的原因。大量的实验和临床研究表明，其发病机制是多方面的，而肾的血流动力学改变在脓毒症性肾损伤的病理生理学机制中扮演了非常重要的角色。本文的目的是综述脓毒症性肾损伤肾灌注的影响因素及相关临床评估方法的研究进展，有助于人们对急性肾损伤的病理生理学改变有一个更加全面的了解，同时能够更准确地早期诊断该疾病，并对其进行更加合理的治疗。

Acute kidney injury ( AKI) in the intensive care unit ( ICU) is commonly caused by severe sepsis or septic shock.It have been demonstrated that multiple mechanisms are involved in the AKI.The changes of renal hemodynamics have played the crucial role in the pathophysiology of septic AKI.The aim of this article is to review the factors that influence the renal perfusion in the septic AKI and the development of the way of the clinical assessment.Hopefully, it would provide a direct approach to understand the pathophysiology of AKI, and help the doctors to make an accurate diagnosis of the complications and subsequently make a rational treatment regimen for AKI patients.
