目的:探讨重症脑卒中患者是否存在肠黏膜屏障功能变化。方法选取重症脑卒中患者67例,按发病部位分为4组,即脑干出血组、脑干梗死组、半球出血组、大面积半球梗死组,分别于患病后24小时、72小时、2周、4周采集静脉血,同时选取健康体检者30例作为对照组,抽取外周血,采用双抗体夹心酶联免疫吸附实验(ELISA)法测定血清二胺氧化酶(Diamine oxidase,DAO)、D-乳酸(D-lactic acid,D-LAC)含量。结果与对照组比较,重症脑卒中组D-LAC、DAO 均明显升高(均 P <0.01)。脑卒中组不同时点比较表明:4组间比较差异均有统计学意义(均 P <0.01)。各组从发病24小时至2周均处于高水平,不同时点比较差异无统计学意义(均 P >0.05)。结论重症脑卒中患者病后24小时内即可出现黏膜屏障功能损伤,并且损害可持续2周或更长时间。

목적:탐토중증뇌졸중환자시부존재장점막병장공능변화。방법선취중증뇌졸중환자67례,안발병부위분위4조,즉뇌간출혈조、뇌간경사조、반구출혈조、대면적반구경사조,분별우환병후24소시、72소시、2주、4주채집정맥혈,동시선취건강체검자30례작위대조조,추취외주혈,채용쌍항체협심매련면역흡부실험(ELISA)법측정혈청이알양화매(Diamine oxidase,DAO)、D-유산(D-lactic acid,D-LAC)함량。결과여대조조비교,중증뇌졸중조D-LAC、DAO 균명현승고(균 P <0.01)。뇌졸중조불동시점비교표명:4조간비교차이균유통계학의의(균 P <0.01)。각조종발병24소시지2주균처우고수평,불동시점비교차이무통계학의의(균 P >0.05)。결론중증뇌졸중환자병후24소시내즉가출현점막병장공능손상,병차손해가지속2주혹경장시간。
Objective To explore the functional changes of intestinal mucosal barrier in patients with severe stroke.Methods A total of 67 patients with severe stroke were enrolled in this study.Based on affected sites,all cases were divided into brainstem hemorrhage group,brainstem infarction group,hemispheric hemorrhage group and massive hemispheric infarction group.Venous blood was collected at 24 hours,72 hours,2 weeks,4 weeks for serum D-LAC and DAO by ELISA.The same detections were conducted in 30 healthy individuals as comparison.Results Compared with control group,both D-LAC and DAO were significantly elevated in patients with severe stroke (P <0.01).There were significant different between sub-groups in different time points (P <0.01).From 24 hours to 2 weeks,both D-LAC and DAO kept at a high level.There were no significant different between different time points ( P >0.05).Conclusion Injury in mucous membrane barrier function occurs in patients with severe stroke in 24 hours after illness onset,and it can last for 2 weeks or longer.
