目的:探析急性脑血管患者的血清心肌酶谱变化及其临床意义。方法入选2013年4月—2014年4月间该院符合标准的急性脑血管患者116例为观察组,均于发病7 d内检测其血清心肌酶谱,即乳酸脱氢酶(LDH)、天门冬氨酸氨基转移酶(AST)、α-羟丁酸脱氢酶(α-HBDH)、肌酸激酶(CK)、心肌型肌酸激酶同功酶(CK-MB),并进行神经功能缺损程度的评分。同时选取116名正常健康人作为对照组进行对比分析。结果观察组患者的AST、LDH、CK、α-HBDH、CK-MB水平变化显著优于对照组,结果差异具有统计学意义(P<0.05);脑梗死组患者的AST、LDH、CK、α-HBDH、CK-MB水平变化和脑出血组相比较,结果差异无统计学意义(P>0.05);有意识障碍组患者的AST、LDH、CK、α-HBDH、CK-MB水平变化和无意识障碍组患者相比较,结果差异具有统计学意义(P<0.05)。结论急性脑血管患者存在着明显的心肌酶谱的改变,临床可以通过检测其血清心肌酶谱对临床病情轻重与预后的判断。

목적:탐석급성뇌혈관환자적혈청심기매보변화급기림상의의。방법입선2013년4월—2014년4월간해원부합표준적급성뇌혈관환자116례위관찰조,균우발병7 d내검측기혈청심기매보,즉유산탈경매(LDH)、천문동안산안기전이매(AST)、α-간정산탈경매(α-HBDH)、기산격매(CK)、심기형기산격매동공매(CK-MB),병진행신경공능결손정도적평분。동시선취116명정상건강인작위대조조진행대비분석。결과관찰조환자적AST、LDH、CK、α-HBDH、CK-MB수평변화현저우우대조조,결과차이구유통계학의의(P<0.05);뇌경사조환자적AST、LDH、CK、α-HBDH、CK-MB수평변화화뇌출혈조상비교,결과차이무통계학의의(P>0.05);유의식장애조환자적AST、LDH、CK、α-HBDH、CK-MB수평변화화무의식장애조환자상비교,결과차이구유통계학의의(P<0.05)。결론급성뇌혈관환자존재착명현적심기매보적개변,림상가이통과검측기혈청심기매보대림상병정경중여예후적판단。
Objective The clinical significance and changes of serum myocardial enzyme spectrum to analysis of acute cerebrovas-cular patients. Methods In patients with acute cerebral vascular methods in our hospital from 2013 April to 2014 April in accor-dance with the standards of the 116 cases of the observation group, were treated within seven days and serum myocardial enzyme spectrum, i.e., lactate dehydrogenase(LDH), aspartate aminotransferase (AST), alpha hydroxybutyrate dehydrogenase (-HBDH), cre-atine kinase (CK), cardiac type creatine kinase isoenzyme(CK-MB), and the degree of neurological deficit score. At the same time, we selected 116 cases of healthy people as control group were compared and analyzed. Results In the observation group, AST, LDH, CK, alpha -HBDH, the change of CK-MB level was better than the control group, the difference was statistically significant (P<0.05); AST, LDH, CK, alpha -HBDH, CK-MB serum levels and cerebral hemorrhage group in patients with cerebral infarction were compared, results no statistical difference (P>0.05);conscious disorder group in patients with AST, LDH, CK,-HBDH, CK-MB level changes of alpha and consciousness group were compared, the difference was statistically significant(P<0.05). Conclusion Acute cerebrovascular patients exist obvious myocardial enzyme spectrum changes, clinical can be detected by their serum my-ocardial enzyme spectrum of clinical severity and the prognosis judgement.
