病情评估是护理重要的工作内容，准确地预测疾病风险，及时地给及有效措施，有利于提高医疗安全。改良早期预警评分及简单临床评分是临床常用的两种评估工具，具有简单易行、效果显著的优点。该研究就国内外应用现状结合文献做一综述。

Condition assessment is important in nursing work. Correctly predicting the risk of disease and timely taking effective measures are conducive to improving the medical safety. Modified early warning score and simple clinical score are two kinds of assessment tools commonly used in clinical practice, which are easy and exercisable but with remarkable effect. This paper sum-marizes the current progress of clinical application of them at home and abroad.
