目的:探讨电视胸腔镜联合肋骨接骨板治疗多发性肋骨骨折合并血胸的疗效。方法2011年1月~2014年5月收治36例多发性肋骨骨折,合并血胸12例,血气胸24例,行电视胸腔镜探查、止血、清除胸腔内血凝块、修补肺损伤,定位肋骨骨折部位,切开复位,并采用肋骨接骨板行肋骨内固定术。结果胸壁畸形均矫正。随访3~12个月,平均8个月,胸部X线片显示肋骨接骨板无松动、断裂,无明显并发症发生,均临床治愈。结论应用胸腔镜联合肋骨接骨板治疗多发性肋骨骨折合并血胸,具有创伤小、操作简便、固定可靠、组织相容性好等优点,且有利于促进骨折愈合和呼吸功能改善,是治疗多发性肋骨骨折合并血胸有效的方法。
目的:探討電視胸腔鏡聯閤肋骨接骨闆治療多髮性肋骨骨摺閤併血胸的療效。方法2011年1月~2014年5月收治36例多髮性肋骨骨摺,閤併血胸12例,血氣胸24例,行電視胸腔鏡探查、止血、清除胸腔內血凝塊、脩補肺損傷,定位肋骨骨摺部位,切開複位,併採用肋骨接骨闆行肋骨內固定術。結果胸壁畸形均矯正。隨訪3~12箇月,平均8箇月,胸部X線片顯示肋骨接骨闆無鬆動、斷裂,無明顯併髮癥髮生,均臨床治愈。結論應用胸腔鏡聯閤肋骨接骨闆治療多髮性肋骨骨摺閤併血胸,具有創傷小、操作簡便、固定可靠、組織相容性好等優點,且有利于促進骨摺愈閤和呼吸功能改善,是治療多髮性肋骨骨摺閤併血胸有效的方法。
목적:탐토전시흉강경연합륵골접골판치료다발성륵골골절합병혈흉적료효。방법2011년1월~2014년5월수치36례다발성륵골골절,합병혈흉12례,혈기흉24례,행전시흉강경탐사、지혈、청제흉강내혈응괴、수보폐손상,정위륵골골절부위,절개복위,병채용륵골접골판행륵골내고정술。결과흉벽기형균교정。수방3~12개월,평균8개월,흉부X선편현시륵골접골판무송동、단렬,무명현병발증발생,균림상치유。결론응용흉강경연합륵골접골판치료다발성륵골골절합병혈흉,구유창상소、조작간편、고정가고、조직상용성호등우점,차유리우촉진골절유합화호흡공능개선,시치료다발성륵골골절합병혈흉유효적방법。
Objective To investigate the curative effect of video-assisted thoracoscopic surgery ( VATS) combined with rib plates in the treatment of multiple rib fractures and hemothorax . Methods A total of 36 patients with multiple rib fractures was treated from January 2011 to May 2014.Among them, 12 patients had hemothorax , and 24 had pneumohemothorax .Under the VATS exploration , hemostasis was performed , intrathoracic blood clots were removed , lung damage was repaired , and the fracture site was positioned and was given open reduction .Internal fixation of ribs with rib plates was carried out . Results The original chest wall deformities were corrected after internal fixation .All the 36 cases were followed up for 3-12 months (mean, 8 months).Chest X-ray examinations showed no loosening or breaking . No significant complications occurred . All patients were clinically cured . Conclusions VATS combined with rib plates in the treatment of multiple rib fractures and hemothorax has advantages of less trauma , simple performance , reliable fixation , and good tissue compatibility . This method promotes the healing of fractures and the improvement of respiratory functions , being an effective therapy for multiple rib fractures and hemothorax .
