目的：评价大黄牡丹汤促进急性阑尾炎术后肠功能恢复的有效性和安全性。方法：计算机检索PubMed、CNKI、WanFangData和VIP数据库(建库—2014.7)，收集大黄牡丹汤联合西药治疗急性阑尾炎的随机对照试验文献。提取资料，应用Cochrane 系统评价手册5.3.3版标准结合Jadad 评分法进行质量评价,使用RevMan5.3.3软件进行统计分析。结果：肠鸣音恢复时间,大黄牡丹汤合抗感染比单纯抗感染平均缩短6.23 h，95%CI（-7.47，-4.98）；术后通气时间，大黄牡丹汤合抗感染比单纯抗感染平均缩短12.48 h，95%CI（-15.09，-9.87）；排便时间，大黄牡丹汤合抗感染比单纯抗感染平均缩短-15.75，95%CI （-20.17，-11.34）。大黄牡丹汤合抗感染比单纯抗感染治疗切口感染发生率低RR=0.28,95%CI(0.13,0.59)。结论：大黄牡丹汤结合抗感染治疗较单纯西医治疗有更好疗效，但相关试验存在一定的方法学问题。
목적：평개대황모단탕촉진급성란미염술후장공능회복적유효성화안전성。방법：계산궤검색PubMed、CNKI、WanFangData화VIP수거고(건고—2014.7)，수집대황모단탕연합서약치료급성란미염적수궤대조시험문헌。제취자료，응용Cochrane 계통평개수책5.3.3판표준결합Jadad 평분법진행질량평개,사용RevMan5.3.3연건진행통계분석。결과：장명음회복시간,대황모단탕합항감염비단순항감염평균축단6.23 h，95%CI（-7.47，-4.98）；술후통기시간，대황모단탕합항감염비단순항감염평균축단12.48 h，95%CI（-15.09，-9.87）；배편시간，대황모단탕합항감염비단순항감염평균축단-15.75，95%CI （-20.17，-11.34）。대황모단탕합항감염비단순항감염치료절구감염발생솔저RR=0.28,95%CI(0.13,0.59)。결론：대황모단탕결합항감염치료교단순서의치료유경호료효，단상관시험존재일정적방법학문제。
Objective To systematically evaluate the effects of Dahuang Mudan decoction in the treatment of intestinal function recovery after the excision of acute appendicitis by using Meta-analysis.Methods Databas?es such as PubMed, CNKI, Wanfangand VIP were searched to collect the randomized controlled trials(RCTs) ,on Dahuang Mudan Decoction combined with western medicine for intestinal function recovery after the excision of acute appendicitis taken from intiation establishment of database to July , 2014. Assessment of methodological quality was performed according to the Cochrane systematic review methods and Jadad score system, and Me?ta-analyses were performed by using RevMan5.3.3 software. Results Compared with single therapy of western medicine, Dahuang Mudan Decoction combined with western medicine for Intestinal function recovery after the excision of acute appendicitis was superior in the recovery time of bowel sounds (WMD:-6.23,95%CI-7.47~4.98,P<0.00001),Postoperative exhaust time (WMD:-12.48,95%CI-20.17~11.34,P<0.00001),Defecation time (WMD:-15.75,95%C1-20.17~11.34,P=0.0005),and Incision infection (RR=0.28,95%C10.13~0.59,P=0.0005). Conclusion Dahuang Mudan decoction combined with western medicine has advantage over single therapy of western medicine, but there are some methodological problems in the studies.
