目的 探讨经末节指骨两侧骨隧道缝合法治疗锤状指畸形的临床疗效.方法 2011年5月至2013年8月,对17例锤状指患者应用经末节指骨基底两侧骨隧道缝合法治疗.其中新鲜锤状指11例,陈旧性锤状指6例.术后近指间关节及掌指关节正常活动,6周后拔除克氏针.结果 术后15例获得随访,2例失访,随访时间为6~ 13个月,平均10个月.按Crawford锤状指疗效评价体系评估:优7例,良6例,可2例;优良率为86.7%.结论 经末节指骨基底两侧骨隧道缝合法固定伸肌腱,可明显纠正锤状指畸形,并发症少,获得良好的关节活动度,是治疗锤状指畸形较有效的方法.

Objective To explore the treatment outcome of mallet finger with the suture technique by bone holes of the distal phalanx.Methods From May 2011 to August 2013,seventeen patients (17 fingers) with a mallet finger deformity were included in this study.The terminal extensor tendon was sutured to bone by drilling holes of the distal phalanx after K-wire fixation.Of these mallet fingers,11 cases were fresh,and 6 were old.After 6 weeks postoperatively,the K-wire was removed for all patients.Active and passive DIP joint exercises were initiated immediately after removal of the K-wire.Results Fifteen patients were follow-up for 6 to 13 months with an average of 10 months.According to Crawford's evaluation criteria,7 cases were excellent,6 good and 2 fair.The overall excellent and good rate was 86.7%.Conclusion Treatment of mallet fingers with the suture technique by bone holes of the distal phalanx could obtain satisfactory results with few complications.It could be an effective method of reconstructing the extensor tendon insertion point for mallet fingers.
