目的 探讨第2足趾趾甲皮瓣再造手指指甲甲床的疗效.方法 2005年6月至2013年11月,我们收治指甲甲床缺损或坏死患者16例16指.伴有中末节皮肤软组织缺损患指12例,采用第2足趾趾甲皮瓣游离移植修复;伴有皮肤软组织缺损者,采用携带第2足趾皮肤软组织的趾甲皮瓣游离移植修复,吻合血管神经,重建感觉血运.结果 术后趾甲皮瓣全部存活,随访6 ~ 36个月,手指屈伸功能满意,指腹两点分辨觉为4~6mm,指甲外观满意.结论 应用第2足趾趾甲皮瓣再造手指指甲甲床疗效满意,可最大限度地恢复手指外观以及功能.

목적 탐토제2족지지갑피판재조수지지갑갑상적료효.방법 2005년6월지2013년11월,아문수치지갑갑상결손혹배사환자16례16지.반유중말절피부연조직결손환지12례,채용제2족지지갑피판유리이식수복;반유피부연조직결손자,채용휴대제2족지피부연조직적지갑피판유리이식수복,문합혈관신경,중건감각혈운.결과 술후지갑피판전부존활,수방6 ~ 36개월,수지굴신공능만의,지복량점분변각위4~6mm,지갑외관만의.결론 응용제2족지지갑피판재조수지지갑갑상료효만의,가최대한도지회복수지외관이급공능.
Objective To evaluate the treatment outcomes of second toe nail flap transfer for reconstruction of nail bed defects in the fingers.Methods From June 2005 to November 2013,nail bed defect or necrosis of 16 fingers in 16 patients were treated.Among them 12 patients had skin and soft tissue defects of the distal and middle phalanx as well.The nail bed defect was repaired with free transplantation of vascularized nail flap from the second toe.For cases with additional soft tissue defects,second toe wrap-around flap along with dorsalis pedis flap was transferred with vascular anastomosis and nerve coaptation to restore blood supply and sensation.Results Postoperatively all the nail flaps and skin flaps survived.The patients were follow-up for 6 to 36 months.Finger flexion and extension function was satisfactory.Two-point discrimination at the finger pulps ranged from 4 to 6 mm.Appearance of the nails was satisfactory.Conclusion Vascularized second toe nail flap transfer is an effective method for reconstruction of finger nail bed defects.It can maximize the restoration of finger appearance and function.
