目的:评价多巴胺静脉泵注在颅内动脉瘤栓塞术麻醉中的效果。方法择期行颅内动脉瘤栓塞术的ASA分级Ⅱ~Ⅲ级患者40例,随机分为多巴胺组和对照组,每组20例。多巴胺组术中静脉泵注多巴胺2~4μg/(kg?min),对照组静脉泵注同等容量生理盐水。每组以丙泊酚静脉泵注4mg/(kg?h),瑞芬太尼6μg/(kg?h)麻醉维持,术中静脉泵注尼莫地平2~4μg/(kg?min),并维持平均动脉压60~75mmHg。观察两组麻醉医师暴露在放射线下的时间,麻醉时间及拔管时间。结果多巴胺组术中麻醉医师暴露在放射线下的时间明显少于对照组(P<0.05),两组麻醉时间及拔管时间无显著差异(P>0.05)。结论多巴胺静脉泵注用于颅内动脉瘤栓塞术麻醉,血流动力学更平稳,能减少麻醉医师暴露在放射线下的时间。

목적:평개다파알정맥빙주재로내동맥류전새술마취중적효과。방법택기행로내동맥류전새술적ASA분급Ⅱ~Ⅲ급환자40례,수궤분위다파알조화대조조,매조20례。다파알조술중정맥빙주다파알2~4μg/(kg?min),대조조정맥빙주동등용량생리염수。매조이병박분정맥빙주4mg/(kg?h),서분태니6μg/(kg?h)마취유지,술중정맥빙주니막지평2~4μg/(kg?min),병유지평균동맥압60~75mmHg。관찰량조마취의사폭로재방사선하적시간,마취시간급발관시간。결과다파알조술중마취의사폭로재방사선하적시간명현소우대조조(P<0.05),량조마취시간급발관시간무현저차이(P>0.05)。결론다파알정맥빙주용우로내동맥류전새술마취,혈류동역학경평은,능감소마취의사폭로재방사선하적시간。
Objective?To?evaluate?the?effectiveness?of?dopamine?in?anesthesia?of?endovascular?embolization?for?intracranial?aneurysm.?Methods?40?ASA?Ⅱ-Ⅲ?patients?of?intracranial?aneurysm,?scheduled?endovascular?embolization,?were?randomly?divided?into?two?groups(n=20?each):?dopamine?group?and?control?group.?Dopamine?was?intravenously?infused?at?2-4?μg/(kg?min)?in?dopamine?group,?while?the?equal?volume?of?normal?saline?was?infused?in?control?group?during?operation.?Anesthesia?of?each?group?was?maintained?by?intravenously?infusing?propofol?4?mg/(kg?h),?remifentanil?6?μg/(kg?h)?and?nimodipinum?was?intravenously?infused??at?2-4?μg/(kg?min),?MAP?was?maintained?60-75?mm?Hg?during?operation.?Then?the?time?anesthesiologists?were?exposed?to?radiation,?the?time?of?anesthesia?and?extubation?were?recorded.?Results The time anesthesiologists were exposed to radiation, was signiifcantly decreased?in?dopamine?group?than?that?in?control?group(P<0.05), there were no signiifcant difference in the time of anesthesia and extubation between two group.?Conclusion?Application?of?dopamine?can?maintain?more?stable?hemodynamics?and?decrease?the?time?anesthesiologists?were?exposed?to?radiation,?in?anesthesia?of?endovascular?embolization?for?intracranial?aneurysm.
