目的:分析老年2型糖尿病(T2DM)患者下肢血管病变多普勒超声特征。方法选择62例老年T2DM患者为观察组,同期健康志愿者62例为对照组,均行双下肢彩色多普勒超声检查,观察老年T2DM患者下肢血管病变的超声表现,比较混合性斑块及纤维性斑块发生率,以及两组的下肢血管内膜厚度及血流参数。结果观察组下肢血管混合性斑块、纤维性斑块及斑块总发生率分别为29.0%、41.9%和70.9%,均明显高于对照组(P<0.05);观察组股动脉及腘动脉内膜厚度(IMT)分别为(0.87±0.25) mm和(0.78±0.18) mm,均明显厚于对照组,但股动脉及腘动脉收缩期峰值流速(PSV)和加速度(Acc)均明显低于对照组,差异均有统计学意义(P<0.05)。结论老年2型糖尿病患者下肢动脉病变发生率高,多普勒频谱参数与健康志愿者比较差异有统计学意义。

목적:분석노년2형당뇨병(T2DM)환자하지혈관병변다보륵초성특정。방법선택62례노년T2DM환자위관찰조,동기건강지원자62례위대조조,균행쌍하지채색다보륵초성검사,관찰노년T2DM환자하지혈관병변적초성표현,비교혼합성반괴급섬유성반괴발생솔,이급량조적하지혈관내막후도급혈류삼수。결과관찰조하지혈관혼합성반괴、섬유성반괴급반괴총발생솔분별위29.0%、41.9%화70.9%,균명현고우대조조(P<0.05);관찰조고동맥급객동맥내막후도(IMT)분별위(0.87±0.25) mm화(0.78±0.18) mm,균명현후우대조조,단고동맥급객동맥수축기봉치류속(PSV)화가속도(Acc)균명현저우대조조,차이균유통계학의의(P<0.05)。결론노년2형당뇨병환자하지동맥병변발생솔고,다보륵빈보삼수여건강지원자비교차이유통계학의의。
Objective To analyze the ultrasonic characteristics of elderly type 2 diabetes mellitus (T2DM) patients with lower extremity vascular disease. Methods Sixty-two elderly patients with T2DM were chosen as the observation group, and 62 healthy volunteers were selected as the control group, which all underwent bilateral lower limbs by color Doppler ultrasonography. The ultrasonic characteristics of lower extremity vascular diseases of the el-derly patients with T2DM were observed. The incidence of mixed plaques and fibrous plaque, lower extremity vascu-lar intimal thickness (IMT), and blood flow parameters of two groups were compared. Results The incidence of mixed plaque, fibrous plaque and plaque in the observation group were 29.0%, 41.9%and 70.9%, significantly higher than those in the control group (P<0.05). The femoral artery and popliteal artery IMT in observation group were (0.87± 0.25) mm and (0.78 ± 0.18) mm, significantly thicker than those in the control group, but the peak systolic velocity (PSV), accelerated speed (Acc) of femoral artery and popliteal artery in the observation group were significantly lower than those in the control group (P<0.05). Conclusion Elderly patients with type 2 diabetes mellitus have significant-ly higher incidence of peripheral arterial disease, and there are significant differences in Doppler spectrum parameters between T2DM patients with lower extremity vascular diseases and health volunteers.
