目的：探讨肿瘤大小对PT4a N0 M0期胃癌患者预后的影响。方法回顾性分析170例行胃癌根治术且术后病理诊断为PT4a N0 M0期的胃癌患者的临床资料。比较不同临界值下肿瘤直径较小患者和肿瘤直径较大患者的生存率，然后分析肿瘤大小与其他临床病理特征的相关性。结果所有患者的肿瘤直径为0．5～18．0 cm，平均直径5．2 cm。当区分肿瘤大小的临界值定为8 cm时，生存率的差异最显著。小肿瘤组和大肿瘤组在性别、肿瘤位置和手术切除类型方面存在显著的差异。小肿瘤组胃癌患者的术后生存率明显高于大肿瘤组。结论肿瘤大小与一些肿瘤相关因子呈正相关，且肿瘤大小是PT4a N0 M0期胃癌的独立预后因素。

Objective To explore the impact of tumor size on prognosis of patients with PT4a N0 M0 gastric cancer.Meth-ods 170 patients who underwent radical gastrectomy were diagnosed with PT4a N0 M0 gastric cancer by postoperative pathology, clinical data of patients were retrospectively analyzed.The survival rates of patients with smaller tumor size and larger tumor size in different optimal cutoff values were compared.Then the correlation between tumor size and other clinicopathologic features was analyzed.Results The tumor size ranged from 0.5 to 18.0 cm (mean 5.2 cm) for all patients.When the threshold value was 8 cm,the difference of survival rates was the most evident.Significant differences were detected in sex,tumor site,and resection type between small size tumor and large size tumor.The survival rate of patients with small size tumor had significantly higher survival rates than patients with large size tumor.Conclusion Tumor size and some tumor-related factors are positively correlated.Tumor size is an independent prognostic factor for PT4a N0 M0 gastric cancer.
