目的：研究新疆汉族、维吾尔族、哈萨克族原发性高血压患者24 h尿微量白蛋白、尿钠/钾排泄比率的差异性。方法入选2010年3月至2015年1月在新疆医科大学第一附属医院高血压科就诊，明确诊断、未服药的原发性高血压患者共878例，其中汉族362例，维吾尔族330例，哈萨克族186例。测定24 h尿白蛋白、尿微量白蛋白、尿钠、钾及肾素、血管紧张素、醛固酮水平，比较三个民族间差异性，并分析尿微量白蛋白与尿钠、钾的相关性。结果三个民族间24 h尿微量白蛋白水平比较，维吾尔族＞汉族＞哈萨克族[（403.2±109.7）mg/24 h vs.（163.5±54.9）mg/24 h vs.（113.1±59.1）mg/24 h，均P＜0.05]；24 h尿钠水平，维吾尔族低于哈萨克族[（158.1±28.5）mmol/24 h vs.（173.9±70.5）mmol/24 h，P＜0.05]。24 h尿钾水平，维吾尔族高于哈萨克族[（42.0±7.4）mmol/24 h vs.（36.7±8.9）mmol/24 h，P＜0.05]。24 h 尿钠/钾比值，维吾尔族<汉族<哈萨克族（3.36±0.23 vs.4.39±1.16 vs.5.36±1.64，均P＜0.05）。哈萨克族患者24 h尿微量白蛋白与24 h尿钠/钾呈负相关（r=－0.204，P＜0.05），与24 h尿钾呈正相关（r=0.381，P＜0.05）。结论新疆维吾尔族原发性高血压患者尿钾及尿钠/钾水平低于哈萨克族及汉族，24 h尿微量白蛋白水平及阳性率高于哈萨克族及汉族，存在民族差异。
목적：연구신강한족、유오이족、합살극족원발성고혈압환자24 h뇨미량백단백、뇨납/갑배설비솔적차이성。방법입선2010년3월지2015년1월재신강의과대학제일부속의원고혈압과취진，명학진단、미복약적원발성고혈압환자공878례，기중한족362례，유오이족330례，합살극족186례。측정24 h뇨백단백、뇨미량백단백、뇨납、갑급신소、혈관긴장소、철고동수평，비교삼개민족간차이성，병분석뇨미량백단백여뇨납、갑적상관성。결과삼개민족간24 h뇨미량백단백수평비교，유오이족＞한족＞합살극족[（403.2±109.7）mg/24 h vs.（163.5±54.9）mg/24 h vs.（113.1±59.1）mg/24 h，균P＜0.05]；24 h뇨납수평，유오이족저우합살극족[（158.1±28.5）mmol/24 h vs.（173.9±70.5）mmol/24 h，P＜0.05]。24 h뇨갑수평，유오이족고우합살극족[（42.0±7.4）mmol/24 h vs.（36.7±8.9）mmol/24 h，P＜0.05]。24 h 뇨납/갑비치，유오이족<한족<합살극족（3.36±0.23 vs.4.39±1.16 vs.5.36±1.64，균P＜0.05）。합살극족환자24 h뇨미량백단백여24 h뇨납/갑정부상관（r=－0.204，P＜0.05），여24 h뇨갑정정상관（r=0.381，P＜0.05）。결론신강유오이족원발성고혈압환자뇨갑급뇨납/갑수평저우합살극족급한족，24 h뇨미량백단백수평급양성솔고우합살극족급한족，존재민족차이。
Objective To study the difference of 24 hours urine protein, urinary potassium/sodium in the patients with essential hypertension of the Kazak, Uyghur, and Han nationality in Xinjiang. Methods A total of 878 cases of essential hypertensives first definite diagnosed in the First Affiliated Hospital of Xinjiang Medical University from March 2010 to January 2015 were recruited. In which, 362 cases was Han nationality, 330 cases was Uyghur and 186 cases was Kazak. 24 hours urinary albumin, urinary microalbuminuria, urinary sodium, urinary potassium and levels of renin, angiotensin, aldosterone were determined and compared among three groups. The correlation of urine protein and urinary potassium, urinary sodium were analyzed. Results The level of 24 hours urinary microalbuminuria of three nationality was in sequence of Uyghur>Han>Kazak [(403.2±109.7)mg/24 h vs. (163.5±54.9)mg/24 h vs. (113.1±59.1)mg/24 h, P<0.05]; The level of 24 hours urinary sodium of Uyghur was lower than Kazak [(158.1±28.5)mmol/24 h vs. (173.9±70.5)mmol/24 h, P<0.05]. The level of 24 hours urinary potassium of Uyghur was higher than potassium [(42.0±7.4)mmol/24 h vs. (36.7±8.9)mmol/24 h, P<0.05]. While 24 hours urinary sodium/potassium was Uyghur<Han<Kazak (3.36±0.23 vs. 4.39±1.16 vs. 5.36±1.64,P<0.05). 24 hours urinary microalbuminuria and 24 hours urinary sodium/potassium was negatively correlated (r=-0.204, P<0.05), but positively correlated to 24 hours urinary potassium (r=0.381, P<0.05) in Kazak patients. Conclusion The level of 24 hours urinary potassium, urinary sodium/potassium of the patients with essential hypertension of Uyghur are lower than the Kazak and Han nationality in Xinjiang, The level and the positive radio of 24 hours urinary microalbuminuria of Uyghur are higher than the Kazak and Han nationality. They have national difference.
