目的探讨彩色多普勒超声在婴幼儿肝移植术后肝动脉检测的意义。方法应用彩色多普勒超声对57例婴幼儿肝移植患者术后进行肝动脉检测,观察有无血流信号,肝动脉收缩期流速(S)、舒张期流速(D)及S/D情况。随访时间最短2个月,最长9个月。收集所有患者的动脉检测数据进行分析。结果52例患儿彩色多普勒超声提示肝动脉血流通畅,未见明显异常;5例患儿分别在术后5~8天内彩色多普勒超声检查于肝门部及肝内均未探及到动脉血流信号,考虑肝动脉血栓形成,3例经血管造影证实、1例经超声造影及增强CT证实,1例超声出现假阳性,血管造影显示肝动脉未见明显异常。彩色多普勒超声对肝动脉检测的诊断符合率达98%。结论彩色多普勒超声为婴幼儿肝移植术后动脉的监测及并发症的早期诊断提供重要的依据。

목적탐토채색다보륵초성재영유인간이식술후간동맥검측적의의。방법응용채색다보륵초성대57례영유인간이식환자술후진행간동맥검측,관찰유무혈류신호,간동맥수축기류속(S)、서장기류속(D)급S/D정황。수방시간최단2개월,최장9개월。수집소유환자적동맥검측수거진행분석。결과52례환인채색다보륵초성제시간동맥혈류통창,미견명현이상;5례환인분별재술후5~8천내채색다보륵초성검사우간문부급간내균미탐급도동맥혈류신호,고필간동맥혈전형성,3례경혈관조영증실、1례경초성조영급증강CT증실,1례초성출현가양성,혈관조영현시간동맥미견명현이상。채색다보륵초성대간동맥검측적진단부합솔체98%。결론채색다보륵초성위영유인간이식술후동맥적감측급병발증적조기진단제공중요적의거。
ObjectiveTo investigate the significance of color doppler ultrasound for detecting hepatic artery in pediatric liver transplantation.Methods Application of color doppler ultrasound to observe the hepatic artery in 57 pediatric after liver transplantation, observe whether the signal of blood flow,systolicvelocity(S),diastolic velocity (D) and S/D. The minimum follow-up time is2 months, the longest is9 months. All patients with artery detection data were collected for analysis.Results No obvious abnormality of hepatic artery was observed in52 cases with color doppler ultrasound. Arterial flow signal could not be detected in5 patients5-8 days after transplantation and hepatic artery thrombosis was considered,in which3 cases were confirmed by digital subtraction angiography(DSA),1 case by contrast-enhanced ultrasound and CT, and1 case was confirmed to be ultrasonic false positive with DSA showed no obvious abnormal hepatic artery. Diagnosis coincidence rate of color doppler ultrasound in hepatic artery detection was98%.Conclusion Color Doppler ultrasound provides an important basis for hepatic artery monitoring and early diagnosis of complication after liver transplantation.
