目的:分析Acutrak无头加压螺钉与AO空心拉力螺钉应用于LetenneurⅠ型Hoffa骨折的生物力学差异。方法选取16根新鲜冷冻成人尸体股骨标本,制备成LetenneurⅠ型Hoffa骨折模型。随机分为Acutrak无头加压螺钉固定的观察组(n=8)、AO空心拉力螺钉固定的对照组(n=8),比较两组骨折间压力及轴向压缩位移、循环负荷位移及极限负荷值。结果(1)观察组模型采用Acutrak无头加压螺钉固定后的骨折块间固定压力大于对照组、骨折块间轴向移动距离小于对照组(P<0.05);(2)观察组模型采用Acutrak无头加压螺钉固定后的各个循环负荷周期位移均明显小于对照组(P<0.05);(3)观察组模型极限负荷值明显大于对照组模型(P<0.05)。结论 Acutrak无头加压螺钉固定LetenneurⅠ型Hoffa骨折的强度与稳定性优于AO空心拉力螺钉,更有利于骨折的早期愈合及肢体功能恢复。

목적:분석Acutrak무두가압라정여AO공심랍력라정응용우LetenneurⅠ형Hoffa골절적생물역학차이。방법선취16근신선냉동성인시체고골표본,제비성LetenneurⅠ형Hoffa골절모형。수궤분위Acutrak무두가압라정고정적관찰조(n=8)、AO공심랍력라정고정적대조조(n=8),비교량조골절간압력급축향압축위이、순배부하위이급겁한부하치。결과(1)관찰조모형채용Acutrak무두가압라정고정후적골절괴간고정압력대우대조조、골절괴간축향이동거리소우대조조(P<0.05);(2)관찰조모형채용Acutrak무두가압라정고정후적각개순배부하주기위이균명현소우대조조(P<0.05);(3)관찰조모형겁한부하치명현대우대조조모형(P<0.05)。결론 Acutrak무두가압라정고정LetenneurⅠ형Hoffa골절적강도여은정성우우AO공심랍력라정,경유리우골절적조기유합급지체공능회복。
Objective To analyze biomechanical comparison of Acutrak headless compression screw and AO cannulated lag screw for Letenneur typeⅠHoffa fracture. Methods Sixteen root fresh frozen adult corpses femurs were selected and prepared into LetenneurⅠHoffa fracture model, which were then randomly divided into observation group with Acutrak headless compression screw (n=8) and control group with AO cannulated lag screw (n=8). Fracture pressure and axial compressive displacement, cyclic load limit load and displacement value were com-pared between the two groups. Results (1) The fixed pressure in the observation group was greater than that in the control group, and axial moving distance between blocks was less than that in the control group (P<0.05). (2) The load cycle displacement of every cycle in the observation group was significantly less than that in the control group (P<0.05). (3) The limit load value in the observation group was significantly greater than that in the control group (P<0.05). Conclusion Acutrak headless compression screw for fixing Letenneur Ⅰ Hoffa fracture type is better than AO cannulated lag screw in strength and stability, which is more conducive to early healing of fracture and recov-ery of limb function.
