目的:观察正常血压高值人群行肱动脉血流介导性舒张功能检测的临床价值。方法本研究分别纳入三组病例,其中A组为Ⅰ级高血压病患者,B组为血压高值的正常人群,C组为健康体检者,三组受试者均为50例。测量受试者肱动脉内径在静息状态下、硝酸甘油舌下含服以及反应性充血状态下的变化,对三组受试者肱动脉内径变化率进行计算并做统计学分析。结果三组受试者肱动脉内径在静息状态下差异均无统计学意义(P>0.05);给予硝酸甘油舌下含服时,三组受试者肱动脉内径变化率比较差异也无统计学意义(P>0.05);反应性充血状态下,A、B两组肱动脉内径变化率低于C组,差异均具有统计学意义(P<0.05),且A组内径变化率更低于B组(P<0.05)。结论对于血压高值的正常人群而言,内皮依赖性血管舒张功能失调现象已然存在,其损伤程度在高血压患者中更为明显,临床对此应予以警惕。

목적:관찰정상혈압고치인군행굉동맥혈류개도성서장공능검측적림상개치。방법본연구분별납입삼조병례,기중A조위Ⅰ급고혈압병환자,B조위혈압고치적정상인군,C조위건강체검자,삼조수시자균위50례。측량수시자굉동맥내경재정식상태하、초산감유설하함복이급반응성충혈상태하적변화,대삼조수시자굉동맥내경변화솔진행계산병주통계학분석。결과삼조수시자굉동맥내경재정식상태하차이균무통계학의의(P>0.05);급여초산감유설하함복시,삼조수시자굉동맥내경변화솔비교차이야무통계학의의(P>0.05);반응성충혈상태하,A、B량조굉동맥내경변화솔저우C조,차이균구유통계학의의(P<0.05),차A조내경변화솔경저우B조(P<0.05)。결론대우혈압고치적정상인군이언,내피의뢰성혈관서장공능실조현상이연존재,기손상정도재고혈압환자중경위명현,림상대차응여이경척。
Objective To investigate the clinical value of detecting brachial artery flow-mediated dilation in people with high-normal blood pressure. Methods The study included three groups:group A with gradeⅠhyperten-sive patients, group B with persons of high-normal blood pressure, and group C with healthy people (n=50 in each group). The changes of brachial artery diameter at rest, after sublingual nitroglycerin administration and reactive hy-peremia were measured. The change rate of brachial artery diameter in three groups were calculated and statistically analyzed. Results There was no statistically significant difference in the brachial artery diameter at rest between three groups (P>0.05);there was also no statistically significant difference between the three groups when given sub-lingual nitroglycerin (P>0.05);in the cases of reactive hyperemia, the change rates of brachial artery diameter in group A and B were lower than that in group C, and the change rate in group A was lower than that in group B. The differenc-es were statistically significant (P<0.05). Conclusion Imbalance of endothelium-dependent vasodilation is found in people with high-normal blood pressure, while the damage degree is more obvious in patients with hypertension. Therefore, imbalance of endothelium-dependent vasodilation should be paid attention to in clinic.
