目的：分析不合理用药，为避免和减少错误用药提供参考。方法依据药品说明书、《中国药典》、《抗菌药物临床应用指导原则》等相关要求，对2014年10月至12月医院静脉用药集中调配(PIVAS)药师对医嘱审核发现的不合理用药进行归纳、分析。结果不合理用药占0.51%，主要问题为溶媒选择或用量不当、药物剂量不当、调配方法不当和配伍禁忌等。结论经药师对医嘱审核，可以及时发现和干预不合理用药，保障临床患者用药安全。
목적：분석불합리용약，위피면화감소착오용약제공삼고。방법의거약품설명서、《중국약전》、《항균약물림상응용지도원칙》등상관요구，대2014년10월지12월의원정맥용약집중조배(PIVAS)약사대의촉심핵발현적불합리용약진행귀납、분석。결과불합리용약점0.51%，주요문제위용매선택혹용량불당、약물제량불당、조배방법불당화배오금기등。결론경약사대의촉심핵，가이급시발현화간예불합리용약，보장림상환자용약안전。
Objective Analysis on irrational drug use, and to provide references for the avoidance and reduction of medication errors.Methods On the basis of drug instructions,“Chinese Pharmacopoeia”,“guiding principles of clinical application of antibacterial”and other relevant requirements, that occurred during 2014 from October to December in the hospital pharmacy intravenous admixture services (PIVAS) of irrational use of drugs by pharmacists on the doctor’s orders the audit,induction analysis .Results The irrational drug use accounted for 0.51%, the main prupose was the solvent selection or improper use, Improper dosage of druges, deployment method and Incompatibility etc.Conclusion Pharmacist advice auditing can be timely found and intervention of irrational drug use, clinical medication safety in the patient by guaranteed.
