目的：分析围绝经期妇女取环难度影响因素，提出预防取环困难的对策。方法调取5家医院围绝经期取环病历报告，将511例取环困难者纳入困难组，331例取环容易者纳入一般组，进行因素分析。结果困难组与一般组患者各项取环影响因素指标对比，差异具有统计学意义(P<0.05)；年龄>45岁、上环时间>11年、乡镇级医院上环、其它宫腔手术史、宫体倾斜、宫铜节育器成为围绝经期妇女取环困难独立风险因素。结论围绝经期妇女取环难度影响因素较多，既有妇女自身因素，也有医源性因素；医院与妇女双方均应提高对IUD的重视，在IUD置入后早期规避风险因素。

Objective to analysis the factors affecting the difficulty of perimenopausal women in the ring, put forward the countermeasure of preventing ring taking difficult. Method take 5 hospitals in menopause to take the ring case report, 511 cases of the ring difficulties were included in a difficult group, 331 cases were included in the general group to take the ring, factor analysis. Results:the difficult group and common group of the patients take the comparison factor index cycles, with significant difference (P<0.05);age>45 years old, the ring time>11 years, Sheung Wan, township level hospitals in other uterine cavity operation history, palace, palace body inclination of copper IUD become perimenopausal women take ring difficulties risk factors. Conclusion there are many perimenopausal women take ring factors affecting the difficulty, both women's own factor, also has the iatrogenic factors;Hospital of women and both sides should pay attention to the importance of IUD, in the early to avoid risk factors after IUD implantation.
