目的：探讨多学科综合呼吸康复对老年中、重度COPD患者的康复效果。方法88例稳定期老年COPD患者随机分为康复组50例和对照组38例，康复组除常规内科药物治疗外均进行12周的多学科综合呼吸康复，对照组只采用常规内科药物治疗。2组患者治疗前及12周时均进行心肺运动试验、BODE指数评分及生存质量评分。结果康复组治疗后的Wmax、VO2max、6-MWD、MMRC、BODE指数、日常生活能力、社会活动情况、抑郁及焦虑心理症状均较康复前有显著改善，差异有统计学意义（P<0.01），以上指标和对照组治疗后对比，差异也具有统计学意义（P<0.05或0.01）。对照组的Wmax、VO2max、BODE指数各项指标以及生存质量各项目评分前、后对比差异均无统计学意义（P>0.05）。结论多学科综合呼吸康复能显著提高老年中、重度COPD稳定期患者的康复效果，改善运动耐量及生存质量，运动训练是综合呼吸康复因素的核心。
목적：탐토다학과종합호흡강복대노년중、중도COPD환자적강복효과。방법88례은정기노년COPD환자수궤분위강복조50례화대조조38례，강복조제상규내과약물치료외균진행12주적다학과종합호흡강복，대조조지채용상규내과약물치료。2조환자치료전급12주시균진행심폐운동시험、BODE지수평분급생존질량평분。결과강복조치료후적Wmax、VO2max、6-MWD、MMRC、BODE지수、일상생활능력、사회활동정황、억욱급초필심리증상균교강복전유현저개선，차이유통계학의의（P<0.01），이상지표화대조조치료후대비，차이야구유통계학의의（P<0.05혹0.01）。대조조적Wmax、VO2max、BODE지수각항지표이급생존질량각항목평분전、후대비차이균무통계학의의（P>0.05）。결론다학과종합호흡강복능현저제고노년중、중도COPD은정기환자적강복효과，개선운동내량급생존질량，운동훈련시종합호흡강복인소적핵심。
[Abstrsct]Objective To explore the effects of multidisciplinary comprehensive respiratory rehabilitation in aged patients with moderate or severe chronic obstructive pulmonary disease(COPD). Methods Eighty-eight cases of aged pa-tients with stable COPD were randomly divided into the rehabilitation group of 50 cases and the control group of 38 cases. The two groups all had routine treatment. Meanwhile, the rehabilitation group received multidisciplinary comprehensive respiratory rehabilitation for 12-weeks. Cardiopulmonary exercise test(CPET), BODE index and quality of life(QOL)be-fore and after treatment were compared. Results After 12-weeks of multidisciplinary comprehensive respiratory rehabili-tation, Wmax, VO2max, 6-MWD, MMRC, BODE index, activities of daily living, social activities, depression score and anxi-ety score were significantly improved in the rehabilitation group than baseline and the control group. Wmax, VO2max, BODE index and QOL score had no significant difference in the control group before and after the study. Conclusion Multidisci-plinary comprehensive respiratory rehabilitation can effectively improve the rehabilitation effect in aged patients with sta-ble moderate or severe COPD, and improve the exercise tolerance and quality of life. Exercise training is the major mea-sure of comprehensive respiratory rehabilitation.
