目的应用腹部侧位X线平片检测维持性血液透析患者腹主动脉钙化(AAC)情况,并探讨其相关因素.方法收集82例维持性血液透析患者的临床资料,通过腹部侧位X线平片检测腹主动脉钙化的发生情况,同时检测患者的血常规、血糖、肾功能、血脂、血钙、血磷及全段甲状旁腺素(iPTH)水平,依据是否发生腹主动脉钙化将患者分成AAC组与非AAC组,进行相关统计学分析.结果 82例血液透析患者中有55例(67.1％)发生腹主动脉钙化,钙化主要发生于L4,并且从L1向L4钙化发生逐渐加重.组间对比分析显示,AAC组患者的年龄较大(P＜0.05)且透析龄更长(P＜0.01),低密度脂蛋白及钙磷乘积水平明显高于非AAC组(P＜0.05),此外原发病为糖尿病肾病患者的腹主动脉钙化发生率高于非AAC组(P＜0.05).结论维持性血液透析患者有较高的腹主动脉钙化发生率,其发生可能与年龄、低密度脂蛋白、钙磷乘积水平、透析龄及原发病为糖尿病等因素有关.
목적 응용복부측위X선평편검측유지성혈액투석환자복주동맥개화(AAC)정황,병탐토기상관인소.방법 수집82례유지성혈액투석환자적림상자료,통과복부측위X선평편검측복주동맥개화적발생정황,동시검측환자적혈상규、혈당、신공능、혈지、혈개、혈린급전단갑상방선소(iPTH)수평,의거시부발생복주동맥개화장환자분성AAC조여비AAC조,진행상관통계학분석.결과 82례혈액투석환자중유55례(67.1％)발생복주동맥개화,개화주요발생우L4,병차종L1향L4개화발생축점가중.조간대비분석현시,AAC조환자적년령교대(P＜0.05)차투석령경장(P＜0.01),저밀도지단백급개린승적수평명현고우비AAC조(P＜0.05),차외원발병위당뇨병신병환자적복주동맥개화발생솔고우비AAC조(P＜0.05).결론 유지성혈액투석환자유교고적복주동맥개화발생솔,기발생가능여년령、저밀도지단백、개린승적수평、투석령급원발병위당뇨병등인소유관.
Objective To investigate related factors of abdominal aortic calcification (AAC) in patients undergoing maintenance hemodialysis by lateral lumbar X-ray.Methods A total of 82 patients undergoing maintenance hemodialysis in our hospital were enrolled in the study.Lateral lumbar X-ray score of abdominal aorta was used to determine AAC.The parameters of renal function,blood glucose,blood lipid,blood calcium,blood phosphorus,serum intact parathyroid hormone (iPTH) were measured and compared between patients with AAC and patients without AAC.Results There were 55 cases of AAC (67.1％) in 82 patients undergoing maintenance hemodialysis.Aortic calcification was most severe in front of L4,the incidence and degree of calcification increased from L1 to L4.The parameters analysis showed that age,dialysis age,low density lipoprotein level and calcium-phosphorus product level in patients with AAC were higher than those in patients without AAC (P＜0.05); the incidence of AAC was higher in maintenance hemodialysis patients with diabetic nephropathy (P＜0.05).Conclusion The incidence of AAC is quite high in patients undergoing maintenance hemodialysis,which is associated with age,low density lipoprotein level,calcium-phosphorus product level,dialysis age and diabetic nephropathy.
