目的探讨Orem自护理论对老年全髋关节置换术后优质护理干预效果的影响.方法采用数字表法将126例行老年全髋关节置换术的患者随机分为对照组和观察组,各63例.对照组采取骨科常规护理;观察组在对照组基础上给予Orem自护理论,根据患者的健康需求、病程给予不同优质护理干预.结果观察组Harris评分、ADL评分显著高于对照组(P＜0.05);观察组护理满意度为98.41％,显著地高于对照组的76.19％ (P＜0.05).结论实施Orem自护理论能够提高行全髋关节置换术后老年患者的优质护理干预措施的效果,可缓解术后患者心理焦虑及抑郁程度,降低并发症发生率,增强患者自理能力,改善髋关节功能,极大提高患者对护理工作的满意度.

Objective To explore the effect of Orem self-care theory for high quality nursing intervention after total hip replacement in the elderly.Methods 126 elderly patients undergoing hip joint replacement were randomly divided into a control group and an observation group,63 cases for each group.The control group receive routine nursing care of department of orthopedics; in addition,Orem self-care theory was applied in the observation group:the patients were given different high quality nursing interventions according to their health needs and disease courses.Results The Harris and ADL scores were significantly higher in the observation group than in the control group (P ＜ 0.05).The nursing satisfaction was significantly higher in the observation group than in the control group (98.41％ vs.76.19％,P ＜ 0.05).Conclusions The implementation of Orem self-care theory for total hip arthroplasty elderly patients with nursing intervention can improve the effect of high quality nursing intervention,relieve the patients' postoperative psychological anxiety and depression level,reduce the incidence of complications,increase the patients' self-care ability,improve the patients' hip joint function,greatly improve the patients' satisfaction to nursing work.
