目的:研究单源单能量CT双能量成像法检测体外泌尿系结石成分的可行性。方法使用单源单能量64排CT分别采集36例尿路结石样本在80KVp及140KVp图像,并分别测量结石样本CT值,根据公式计算出DEI(dual energy index,DEI)。结果纯尿酸结石DEI=(0.01230±0.00412),混合尿酸结石DEI=(0.03613±0.01255),含钙结石DEI=(0.09901±0.01553),鸟粪石DEI=(0.06176±0.00650),胱氨酸结石DEI=(0.05825±0.00774)。纯尿酸结石和非纯尿酸结石之间DEI值有统计学差异(P<0.05);纯尿酸、混合尿酸、含钙结石之间DEI均有统计学差异(P<0.05);鸟粪石、胱氨酸结石之间和它们与混合尿酸、含钙结石之间DEI没有统计学差异。结论单源单能量CT双能量成像法是一种方便、快捷的方法,可在没有双源双能量CT及能谱CT的情况下明确泌尿系结石的成分,对临床上进一步的诊断及治疗方案的选择是非常有价值的。

Objective To study the feasibility of single-source single-energy CT dual energy imagine for stone composition in vitro.Methods Using single-source single-energy 64 CT, imagines of 36 stones were collected by 80KVP and 140 KVP respectively, and CT values were measured. Then DEI values were calculated according to the certain formula.Results The DEI values were (0.01230±0.00412) (UA), (0.03613±0.01255) (mixed UA), (0.09901±0.01553) (calcified stones), (0.06176±0.00650) (struvite stones), (0.05825±0.00774) (cysteine stones). The DEI values between UA-containing and no UA-containing were statistically significant (P<0.05), the DEI values were statistically significant among UA, mixed UA and calcified stones (P<0.05); there were no statistically significant among struvite , cysteine , mixed UA and calcified stones.Conclusion Single-source single-energy CT dual-energy imaging method for stone composition is convenient, rapid and non-invasive . It can provide valuable information for clinical diagnosis and treatment design.
