目的:探讨鲍氏不动杆菌在临床标本中的分布及其耐药性,为指导临床合理用药和控制医院感染提供依据。方法对2009-2012年临床分离各类标本中的鲍氏不动杆菌分布及药敏结果进行统计分析,按《全国临床检验操作规程》对菌株进行鉴定,采用CLSI推荐的K‐B纸片法进行药敏试验,采用WHONTE5.4软件进行数据统计。结果共分离出革兰阴性菌5796株,其中鲍氏不动杆菌635株,检出率为11.0%,呼吸道痰标本占89.8%;主要集中在IC U、内科、外科等;鲍氏不动杆菌对头孢哌酮/舒巴坦耐药率最低,<8.0%,其次为多黏菌素B、黏菌素、亚胺培南、美罗培南,耐药率<25.0%,对其他抗菌药物耐药率较高。结论鲍氏不动杆菌对多种抗菌药物均产生不同程度的耐药性,临床应加强其耐药性监测及针对性管理措施,减少耐药性菌株的产生及扩散。

목적:탐토포씨불동간균재림상표본중적분포급기내약성,위지도림상합리용약화공제의원감염제공의거。방법대2009-2012년림상분리각류표본중적포씨불동간균분포급약민결과진행통계분석,안《전국림상검험조작규정》대균주진행감정,채용CLSI추천적K‐B지편법진행약민시험,채용WHONTE5.4연건진행수거통계。결과공분리출혁란음성균5796주,기중포씨불동간균635주,검출솔위11.0%,호흡도담표본점89.8%;주요집중재IC U、내과、외과등;포씨불동간균대두포고동/서파탄내약솔최저,<8.0%,기차위다점균소B、점균소、아알배남、미라배남,내약솔<25.0%,대기타항균약물내약솔교고。결론포씨불동간균대다충항균약물균산생불동정도적내약성,림상응가강기내약성감측급침대성관리조시,감소내약성균주적산생급확산。
OBJECTIVE To explore the distribution and drug resistance of the Acinetobacter baumannii strains isola‐ted from clinical specimens so as to provide guidance for reasonable clinical use of antibiotics and the control of nosocomial infections .METHODS From 2009 to 2012 ,the distribution of the A .baumannii strains isolated from the clinical specimens and the results of drug susceptibility testing w ere statistically analyzed , then the strains were identified according to national clinical laboratory procedures ,the drug susceptibility testing was performed by using K‐B disk method recommended by the CLSI ,and the statistical analysis was carried out with the use of WHONTE software .RESULTS A total of 5 796 strains of gram‐negative bacteria have been isolated ,of which 635 were the A .baumannii ,with the detection rate of 11 .0% ,89 .8% of the strains were isolated from respiratory sputum specimens ,and strains dominated in the ICU ,department of internal medicine ,and department of surger‐y .The drug resistance rate of the A .baumannii to cefoperazone‐sulbactam was less than 8 .0% ,the drug resistance rates to polymyxin B ,colistin ,imipenem ,and meropenem were less than 25 .0% ,and the drug resistance rates to other antibiotics were high .CONCLUSION The A .baumannii strains show various degree of drug resistance to multiple antibiotics ,thus ,it is necessary for the hospital to strengthen the surveillance of drug resistance and take targeted management measures so as to reduce the drug‐resistant strains .
