目的：通过对腰椎滑脱患者手术后的临床研究，分析其术后感染的相关危险因素，为临床正确治疗、合理使用抗菌药物，降低医院感染率提供科学依据。方法采用回顾性研究方法，对2011年1月－2013年12月入住骨科的216例腰椎滑脱患者进行研究，对其术后感染情况进行分析，采用SPSS软件对数据进行统计分析。结果216例腰椎滑脱患者术后8例发生感染，感染率为3．7％，其中手术切口浅部感染5例，占62．5％，另外3例患者出现深部感染，占37．5％；共检出病原菌8株，其中革兰阳性菌6株占75．0％，革兰阴性菌2株占25．0％；手术时间长、出血量多、患者年龄高、伴随疾病重、病因类型复杂、外力负荷大及病程长，发生感染风险越高，感染程度也越重。结论积极治疗患者伴随慢性疾病，明确腰椎滑脱病因，认真掌握手术适应证，熟练手术操作，缩短手术时间，密切感染监控，合理使用抗菌药物，可减少腰椎滑脱术后感染的发生。

목적：통과대요추활탈환자수술후적림상연구，분석기술후감염적상관위험인소，위림상정학치료、합리사용항균약물，강저의원감염솔제공과학의거。방법채용회고성연구방법，대2011년1월－2013년12월입주골과적216례요추활탈환자진행연구，대기술후감염정황진행분석，채용SPSS연건대수거진행통계분석。결과216례요추활탈환자술후8례발생감염，감염솔위3．7％，기중수술절구천부감염5례，점62．5％，령외3례환자출현심부감염，점37．5％；공검출병원균8주，기중혁란양성균6주점75．0％，혁란음성균2주점25．0％；수술시간장、출혈량다、환자년령고、반수질병중、병인류형복잡、외력부하대급병정장，발생감염풍험월고，감염정도야월중。결론적겁치료환자반수만성질병，명학요추활탈병인，인진장악수술괄응증，숙련수술조작，축단수술시간，밀절감염감공，합리사용항균약물，가감소요추활탈술후감염적발생。
OBJECTIVE To investigate the related risk factors for postoperative infections in the patients with lum‐bar spondylolisthesis so as to provide guidance for correct clinical treatment and reasonable use of antibiotics as well as the reduction of incidence of nosocomial infections .METHODS By means of retrospective survey ,totally 216 patients with lumbar spondylolisthesis who were hospitalized the department of orthopedics from Jan 2011 to Dec 2013 were enrolled in the study ,then the prevalence of postoperative infections was analyzed ,and the statisti‐cal analysis was performed with the use of SPSS software .RESULTS The postoperative infections occurred in 8 of 216 patients with lumbar spondylolisthesis ,with the infection rate of 3 .7% ;of the 8 cases of infections ,5 had the superficial incision infections ,accounting for 62 .5% ,and 3 had the deep infections ,accounting for 37 .5% .A total of 8 strains of pathogens have been isolated ,including 6 (75 .0% ) strains of gram‐positive bacteria and 2 (25 .0% ) strains of gram‐negative bacteria .The long operation duration ,heavy bleeding volume ,advanced age ,complication of severe disease ,complex cause of disease ,large external load ,and long disease course were the risk factors for the infections ,and the infections were more severe in the patients who had the above risk factors .CONCLUSION It is an effective way to actively treat the complicated chronic disease ,define the causes of lumbar spondylolisthesis , grasp the surgical indications ,make the surgical procedures proficient to shorten the operation duration ,closely monitor the infections ,and reasonably use antibiotics so as to reduce the incidence of postoperative infections in the patients with lumbar spondylolisthesis .
