目的:探讨对于不典型胸痛患者通过运动负荷实验激发心肌缺血,观察NT-proBNP的变化与冠脉病变的关系。方法观察2013年1~11月选择住院患者60例临床因胸痛而行运动实验阳性的患者为观察组,同时选取同期阴性患者60例为对照组,进行运动负荷实验,阳性患者在运动前及运动后120min进行NT-proBNP的检查,最后择期行冠脉造影检查。结果观察组患者的NT-proBNP高于对照组、Gensini评分低于对照组,两组比较差异有统计学意义(P<0.05);而LVEF水平观察组低于对照组,两组比较差异有统计学意义(P<0.05)。观察组患者进行冠脉造影,然后根据分支观察NT-proBNP水平,冠脉病3支的NT-proBNP水平明显高于2支及1支者,差异有统计学意义(P<0.05)。60例运动负荷实验阳性患者NT-proBNP检测水平明显升高,均行冠脉造影检查提其中54例提示冠脉病变,2例行支架治疗,阳性率90%。结论运动负荷实验阳性的患者,运动前后NT-proBNP明显升高者,表明冠脉病变较严重。

목적:탐토대우불전형흉통환자통과운동부하실험격발심기결혈,관찰NT-proBNP적변화여관맥병변적관계。방법관찰2013년1~11월선택주원환자60례림상인흉통이행운동실험양성적환자위관찰조,동시선취동기음성환자60례위대조조,진행운동부하실험,양성환자재운동전급운동후120min진행NT-proBNP적검사,최후택기행관맥조영검사。결과관찰조환자적NT-proBNP고우대조조、Gensini평분저우대조조,량조비교차이유통계학의의(P<0.05);이LVEF수평관찰조저우대조조,량조비교차이유통계학의의(P<0.05)。관찰조환자진행관맥조영,연후근거분지관찰NT-proBNP수평,관맥병3지적NT-proBNP수평명현고우2지급1지자,차이유통계학의의(P<0.05)。60례운동부하실험양성환자NT-proBNP검측수평명현승고,균행관맥조영검사제기중54례제시관맥병변,2례행지가치료,양성솔90%。결론운동부하실험양성적환자,운동전후NT-proBNP명현승고자,표명관맥병변교엄중。
Objective To explore the clinical analysis of the relationship between NT-proBNP changes and coronary artery lesion before and after exercise test positive patients with chest paln movement. Methods 60 inpatients in clinical for patients with chest paln along motion test positive from 2013 January to 2013 year in November to observe the selection in for the observation group, select the same period and negative in 60 cases as control group, exercise load experiment, positive patients in the movement before and after exercise for 120 minutes to check NT-proBNP, finally elective coronary angiography. Results The patients in the observation group were higher than those in the control group, the NT-proBNP Gensini were lower than those of control group, the two groups, the difference was statistically significant (P<0.05);while the LVEF bottle in observation group than in the control group, the two groups, the difference was statistically significant (P<0.05). The observation group patients underwent coronary angiography, then according to the branch of study on the level of NT-pro-BNP, coronary disease 3 support NT-proBNP level obviously is higher than 2 and 1, the difference was statistically significant (P<0.05). 60 cases of patients with positive exercise load test detection of NT-proBNP level increased significantly, underwent coronary angiography with 54 cases prompt coronary lesions, 2 patients underwent stent treatment, the positive rate was 90%. Conclusion Exercise load test positive patients before and after exercise, NT-proBNP increased significantly, showed that coronary artery disease is serious.
