目的 对比组织多普勒(TDI)-Tei指数、血流脉冲多普勒(PW)-Tei指数、M超声法所测左心室射血分数(LVEFM)及Simpson法所测左心室射血分数(LVEsimpson),评价慢型克山病(CKD)患者的左心室收缩功能,为评估CKD患者的左心室收缩功能寻找准确、简便的超声学指标.方法 2012年,在山东省克山病病区泗水、邹城、莒县、五莲、沂水和青州6个县(市)选择35例CKD患者(CKD组)作为研究对象;32例健康体检人员作为对照组.对调查对象进行二维超声心动图及TDI检查,测量并计算TDI-Tei、PW-Tei、LVEFM及LVEsimpson,比较CKD组和对照组4个指数的组间差异,分析4个指数间的相关性.结果 CKD组患者TDI-Tei指数、PW-Tei指数(0.87±0.30、0.70±0.32)均高于对照组(0.44±0.21、0.38±0.22,t=6.84、4.80,P均<0.01),LVEFM、LVEFsimpson[(37.37±10.41)%、(35.51±8.43)%]均低于对照组[(65.25±12.92)%、(63.38±13.13)%,t=-9.67、-10.23,P均<0.01].CKD组和对照组中的TDI-Tei、PW-Tei、LVEFM与LVEFsimpson指数的相关系数分别为-0.726、-0.575、0.697,-0.735、-0.686、0.785,P均<0.01).结论 TDI-Tei指数较LVEFM、PW-Tei指数能更准确地评价CKD患者左心室收缩功能,操作简便,可在流行病学调查和临床工作中推广使用.

Objective To evaluate the left ventricular systolic function in patients with chronic Keshan disease (CKD) by 4 methods of tissue Doppler imaging Tei (TDI-Tei) index,pulsed wave Doppler Tei (PW-Tei) index,M type ultrasonic of left ventricular ejection fraction (LVEFM) and Simpson method of left ventricular ejection fraction (LVEFsimpson) indexes.To look for a simple and reliable index of echocardiography for left ventricular systolic function.Methods In 2012,35 patients with CKD were randomly sampled as CKD group.They were from 6 Keshan disease districts in Shandong Province,Sishui,Zoucheng,Wulian,Juxian,Yishui and Qingzhou.Totally 32 healthy people were selected as control group.Traditional echocardiography and TDI were employed for evaluation of the cases.The indexes of TDI-Tei,PW-Tei,LVEFM and LVEFsimpson were measured and difference was compared between groups.Correlation was analyzed among the 4 indexes.Results TDI-Tei and PW-Tei of the CKD patients (0.87 ± 0.30,0.70 ± 0.32) were significantly higher than those of the control group (0.44 ± 0.21,0.38 ± 0.22,t =6.84,4.80,all P < 0.01),whereas LVEFM and LVEFsimpson of the CKD patients [(37.37 ± 10.41)%,(35.51 ± 8.43)%] were significantly decreased than those of the control group [(65.25 ± 12.92)%,(63.38 ± 13.13)%,t =-9.67,-10.23,all P < 0.01].In CKD group the correlation coefficients for TDI-Tei and LVEFsimpson,PW-Tei and LVEFsimpson,LVEFM and LVEFsimpson were-0.726,-0.575 and 0.697 (all P < 0.01),while in the control group were-0.735,-0.686 and 0.785 (all P < 0.01).Conclusions TDI-Tei index could more accurately evaluate left ventricular systolic function than LVEFM and PW-Tei index in CKD patients.It has operated simply and is worth applying in the epidemiologic survey and clinical practice.
