目的:采用正交试验设计研究影响针刺效应的因素,探索影响针刺效应的主次因素和针刺参数的最佳组合,以期为提高针刺疗效、指导针灸临床提供实验依据。方法氢化可的松肌肉注射造皮质酮肾阳虚大鼠模型。采用L9(34)正交表对影响针刺效应的4种因素(穴位、捻针角度、捻针频率、捻针时间)3水平安排试验,运用计算机控制的针刺手法仪治疗肾阳虚大鼠,通过ELISA检测血清中T3、促甲状腺激素( TSH )的水平和 RCT-PCR 检测下丘脑组织中促甲状腺激素释放激素( TRH) mRNA的表达作为考察指标。结果对针刺效应的影响大小依次是捻针频率、捻针时间、腧穴选择、捻针角度。捻针频率是影响针刺效应的主要因素( P<0.01);捻针时间是影响针刺效应的主要因素或重要因素( P<0.01或P<0.05);穴位是影响针刺效应的重要因素( P<0.05);捻针角度是影响针刺效应的次要因素( P>0.05)。治疗肾阳虚大鼠的最佳针刺参数为刺肾俞穴,捻针角度为90°,频率为120次/min,捻针时间60 s或120 s。结论捻针频率和捻针时间是影响针刺效应的主要因素或重要因素,是构成针刺刺激量的主--素。

Objective To study the operational parameters of acupuncture treatment for rats model of kidney-yang dificiency syndrome, with serum T3, TSH and hypothalamic TRH mRNA as indicators, and to find the main influencing factors and an optimal combination of factors so as to provide experimental evidence. Methods Rat model of kidney-yang deficiency syndrome was induced by intramuscular injection of hydrocortisone. L9 (34 ) orthogonal experiment was designed with four factors viz. acupoint, angle of twisting-rotation ( angle ) , frequency of twisting-rotation ( frequency ) , duration of twisting-rotation ( duration) at three levels respectively. The rats were treated by acupuncture manipulation device controlled by computer. Serum T3 and TSH were measured by using ELISA, and the expression of hypothalamic TRH mRNA was determined by using RT-PCR. Results Frequency was the primary factor (P<0. 01), duration was the secondary factor(P< 0. 01 or P<0. 05) and acupoint was the tertiary factor (P < 0.05 ). The optimal acupuncture factors were acupoint of Shenshu (BL23) , angle of 90°, frequency of 120 times/min and duration of 60 s or 120 s. Conclusion Frequency and duration were the essential factors of intensity of acupuncture, which were also the important factors to influence of acupuncture.
