目的探讨吻合血管的(足母)(趾)甲瓣移植修复拇、手指甲床缺损的临床效果. 方法 2010年2月-2014年2月,采用吻合血管的(足母)(趾)甲瓣移植修复拇指甲床缺损18例,采用吻合血管的第2趾甲瓣移植修复手指甲床缺损10例,采用吻合血管的部分(足母)甲瓣移植修复手指甲缺损7例.足部供区采用皮片植皮或足背筋膜皮瓣或游离皮瓣修复. 结果 35例移植的(足母)(趾)甲瓣均获得成活,足趾供区愈合良好.术后随访6～ 24个月,平均10个月,拇、手指指甲血运良好,色泽红润,外形良好,有光泽,指甲生长平整,可协助指腹完成抓、掐、捡针等动作.供区无瘢痕挛缩,无疼痛,足趾行走功能无影响. 结论应用吻合血管的(足母)(趾)甲瓣移植修复拇手指甲床缺损临床效果良好.

Objective To explore the effect of wrap-around transplantation flap with blood supply for repairing thumb or finger nail bed defect.Methods From February,2010 to February,2014,the thumb or finger nail bed defect were repaired with wrap-around flap from the big toe (18 cases),from the second toe (10 cases) andfrom the part of the big toe (7 cases).All transplantation were carried out with blood supply.The donor sites of thefoot were repaired with skin graft or dorsal fascial flap or free flap.Results All 35 transplanted wrap-around flap were all survived,the donor area of foot healed well.All cases were followed-up for 6-24 months (average 10 months),thumb and finger nail had good blood circulation,ruddy color,and good appearance.The nail growth smoothly,and could help the finger pulp to grab,pinch,pick up needle and other actions.The donor area had no scar contracture,no pain,and no influence of toe in walking function.Conclusion Wrap-around flap transplantation with blood supply has good clinical effect for repairing thumb or finger nail bed defect.
