目的：对单枚cage和单枚cage加单侧椎弓根螺钉内固定术治疗腰椎间盘突出并腰椎不稳症安全性进行探讨和分析。方法选取我院于2011年12月～2013年12月收治的50例腰椎间盘突出并腰椎不稳症患者，分成对照组和观察组，每组各25例，对照组患者采用单枚cage植骨融合术进行治疗，观察组患者采用单枚cage加单侧椎弓根螺钉内固定术进行治疗，记录患者的手术时间、术中出血量等，同时对患者进行为期一年的随访，观察患者的治疗效果，以及并发症发生情况。结果两组患者在手术时间、术中出血量等方面差异不明显（P>0.05），通过对患者进行随访，观察组患者优良率较对照组高，在并发症方面，对照组中有7例患者出现cage脱出，观察组无，两组差异具有统计学意义（P<0.05）。结论针对腰椎间盘突出并腰椎不稳症患者，单枚cage加单侧椎弓根螺钉内固定术具有较好的治疗效果，降低患者术后并发症。

목적：대단매cage화단매cage가단측추궁근라정내고정술치료요추간반돌출병요추불은증안전성진행탐토화분석。방법선취아원우2011년12월～2013년12월수치적50례요추간반돌출병요추불은증환자，분성대조조화관찰조，매조각25례，대조조환자채용단매cage식골융합술진행치료，관찰조환자채용단매cage가단측추궁근라정내고정술진행치료，기록환자적수술시간、술중출혈량등，동시대환자진행위기일년적수방，관찰환자적치료효과，이급병발증발생정황。결과량조환자재수술시간、술중출혈량등방면차이불명현（P>0.05），통과대환자진행수방，관찰조환자우량솔교대조조고，재병발증방면，대조조중유7례환자출현cage탈출，관찰조무，량조차이구유통계학의의（P<0.05）。결론침대요추간반돌출병요추불은증환자，단매cage가단측추궁근라정내고정술구유교호적치료효과，강저환자술후병발증。
Objective To discuss and analysis safety of single cage and single cage plus unilateral pedicle screw fixation in the treatment of lumbar disc herniation complicated with lumbar instability syndrome. Methods 50 lumbar disc herniation complicated with lumbar instability syndrome patients who were treated in our hospital from December 2011 to December 2013 were randomized into an observation group and a control group. The control group were treated by bone grafting with single cage treatment,and the observation group were treated with fixation of single cage plus unilateral pedicle screw. Recorded the operative time of patients intraoperative blood loss,etc.,at the same time to a one-year follow-up of patients,observed the therapeutic effect of patients,and complications. Results The two groups of patients in the operative time,amount of bleeding and other aspects of the difference was not statistically significant(P>0.05). According to one-year follow-up the patients in observation group were excellent and good rate was higher than the control group. In the complications,the control group had 7 patients appeared cage prolapse,and the observed group didn't. And the difference was statistically significant(P<0.05). Conclusion For lumbar disc herniation complicated with lumbar instability syndrome patients. Single cage plus unilateral pedicle screw fixation is worth to be promoted and applied which has better treatment effect and reduce rate of complication.
