目的：分析处方点评工作中暴露的问题，促进临床合理用药。方法以随机抽样法抽取该院每月门诊处方100张，共计1200张，临床药师对处方逐一进行审查，对不合理用药处方进行统计分析，并对其中的部分典型不合理处方进行点评。结果不合理处方122张，占总抽查处方数的10．17％，不合理处方类型主要包括临床诊断不明确、药物用法、用量不合理、溶媒选择或合用不合理、无指征使用抗生素、药物配伍不当等。结论医院用药基本合理，但仍有小部分处方用药不合理，需大力改进；处方点评是了解临床用药水平的重要手段，通过定期进行处方点评，剖析临床用药存在的问题，及时发现，合理干预，将有利于提高临床医师合理用药水平。
목적：분석처방점평공작중폭로적문제，촉진림상합리용약。방법이수궤추양법추취해원매월문진처방100장，공계1200장，림상약사대처방축일진행심사，대불합리용약처방진행통계분석，병대기중적부분전형불합리처방진행점평。결과불합리처방122장，점총추사처방수적10．17％，불합리처방류형주요포괄림상진단불명학、약물용법、용량불합리、용매선택혹합용불합리、무지정사용항생소、약물배오불당등。결론의원용약기본합리，단잉유소부분처방용약불합리，수대력개진；처방점평시료해림상용약수평적중요수단，통과정기진행처방점평，부석림상용약존재적문제，급시발현，합리간예，장유리우제고림상의사합리용약수평。
Objective To analyze the problems noted in prescription comment so as to promote clinical rational drug use.Methods A total of 1200 outpatient prescriptions (100 per month) were randomly collected for analysis and comment. Results 122 (10.17%) outpatient prescriptions were irrational.Irrational prescription types include unclear clinical diagno-sis,drug usage,dosage,solvent selection or combination of unreasonable unreasonable,no indications for use of antibiotics,drug incompatibility etc.Conclusion Basic reasonable to hospital medicine,but there are still number of prescription are not rea-sonable to drug use,which need to improve.Prescription comment is an important means for probing into the clinical rational drug use level.The rational drug use level can be improved through regular evaluation and analysis of the problems of clinical drug use,timely detection of problems and proper intervention.
